联合检测外周血AFP mRNA、h-TERT mRNA对肝细胞肝癌的诊断价值

摘　　要：目的探讨肝细胞肝癌患者外周血AFP mRNA、h-TERT mRNA表达对肝细胞肝癌的诊断价值。方法应用荧光定量聚合酶链反应(FQ-PCR)定量检测60例肝细胞肝癌患者外周血AFP mRNA、h-TERT mRNA的表达量,并与对照组进行比较。结果肝细胞肝癌患者外周血中的AFP mRNAh、-TERT mRNA表达量比对照组增高,差异有统计学意义(P<0.01)。它们的阳性率分别达到76.7%、83.3%,和对照组比较差异有统计学意义(P<0.01);联合两个指标进行检测,敏感度提高到92.6%,同时其特异度为97.2%,是诊断肝细胞肝癌的良好肿瘤标志物。其表达结果与肿瘤包膜是否完整、有无肝内浸润和(或)转移明显相关(P<0.05);而与肝癌组织的分化程度、外周血AFP的表达差异无统计学意义(P>0.05)。结论在外周血中定量检测AFP mRNA、h-TERT mRNA表达量可以作为肝细胞肝癌的诊断指标,能够早期发现肿瘤,甚至可以发现其他检测方法尚不能发现的小肝癌,联合检测的意义更为明显。Objective To study the value of comhined detection of the serum levels ot AFP mRNA and h-TERI mRNA m the early diagnosis of hepatoeellular carcinoma. Methods The serum levels of AFP mRNA and hTERT mRNA in 30 patients with hepatoeellular carcinoma and 35 controls were measured by real-time quantitative PCR,then the correlation between the expression of h-TERT mRNA,AFP mRNA and the malignant nature of HCC was analyzed. Results AFP mRNA,h-TERT mRNA expression value in peripheral blood of patients with hepatocellular carcinoma were higher than the control group significantly（P〈0.01）. The positive rate of AFP mRNA, h-TERT mRNA in hepatocellular carcinoma group were 76.70% ,83.3 %, respectively. They were sig- nificantly higher than those in control group（P〈0.01）. But there were no expression in control group in AFP mRNA and there was appostive case in h-TERT mRNA in control group, Sensitivity of combined detection of two kinds of tumormarkers was 92. 6 % ,The specificity of this experiment was 97.2 %. It was significantly higher than that of single detection. Conclusion Combina- tion of detection of AFP mRNA.h-TERT mRNA in peripheral blood is a sensitive and valuable assay for the early prediction of the prognosis of the HCC patients. And it can also help to distinguish the potential patients.
