摘  要:目的研究大鼠在平均动脉压(MAP)为40mmHg时不同时间点有关生理生化变化和病理改变。方法将24只健康SD大鼠随机分成正常对照组(N)与实验组,其实验组将大鼠放血制成MPA为40mmHg的休克模型,根据不同时间分成30分钟组(M30),60分钟组(M60),90分钟组(M90)。每组6只大鼠。每组记录心率、呼吸频率、动脉血pH、剩余碱、血乳酸,并做心、脑的病理切片。结果 M30组心率、呼吸频率增快与N组比较P>0.05;pH,剩余碱、乳酸明显变化(P<0.05);心肌纤维及脑组织轻度肿胀。M60组心率、呼吸频率、剩余碱、乳酸相比对照组及M30组均有明显变化(P<0.05),心肌细胞横纵纹不清晰,局部片状出血,血管扩张淤血,局部片状出血;脑组织肿胀明显,部分胶质细胞增生,脑血管扩张。M90组心率、呼吸频率、剩余碱、乳酸变化更加明显与N组和M30组比较P<0.05,心肌局部肌纤维排列紊乱,正常结构消失;脑组织肿胀明显,部分细胞变性坏死。结论大鼠当MAP为40mmHg时,在30分钟内心率、呼吸增快;血乳酸、剩余碱明显变化,但心、脑只有轻微的病理改变。60分钟后机体失去代偿能力,心、脑病理改变明显。Objective To study the physiological,biochemical and pathological changes observed at different time points with mean arterial pressure(MPA) being 40mmHg.Methods A total of 24 healthy SD rats were randomly divided into control group and experimental group.Depending on different time points,the experimental group was subdivided into 30 minutes group(M30),60 minutes group(M60) and 90 minutes group(M90),with 6 rats in each group.Heart rate,respiratory rate,pH value,base excess and blood lactate were recorded and pathological section of heart and brain was collected for examination.Results Compared with control group,rats in M30 group had features of higher-frequency of heart rate and respiratory rate,obvious changes in pH value,base excess and lactic acid,and slight swelling in the myocardial fiber and brain tissue.Rats in M60 group were found: there was obvious change in heart rate,respiratory rate,base excess,lactic acid(P0.05);the myocardial cell was observed with unclear horizontal-vertical striation,bleeding in partial slice,dilatation,congestion and hemorrhage in blood vessels,obvious swelling in brain tissue,proliferation in some glial cells,dilatation in blood vessels.Rats in M90 group indicated: more obvious changes in heart rate,respiratory rate,base excess,lactic acid changes(P0.05);derangement in partial myocardial and normal structure disappeared;obvious swelling was found in brain tissue,some of which were degenerated and necrosed.Conclusion At the time of MAP being 40 mmHg,body can compensate and there are slight pathological changes in heart and brain within 30 minutes.The body will decompensate,and there are obvious pathologic changes after 60 minutes.
