摘  要:目的评价应用A超分段测量法对硅油填充眼进行生物测量的准确性。方法前瞻性选取2008年1月至12月间在我院行硅油取出术的硅油填充眼患者29例(29眼),在术前、术后3个月均用眼科A超测量眼轴长度并进行比较;其中硅油填充眼的A超分段测量法采用仰卧位接触法测量,硅油泡长度采用1532m.s-1的声速值测得表观长度后乘以0.644,得到硅油泡的实际长度,并考虑到硅油泡下水层的厚度,即计算公式为:硅油填充眼的眼轴长度=前房深度+晶状体厚度+0.644×硅油泡的表观长度+硅油泡下水层的厚度。结果用眼科A超分段测量法测得术前、术后的眼轴长度平均为(26.11±2.85)mm、(26.06±2.80)mm;同一患眼术前、术后眼轴长度的差值平均为(0.06±0.27)mm,术前、术后测得结果高度相关,相关系数为0.995,差异无统计学意义(P=0.283>0.05);患眼术后与对侧眼眼轴长度的差值水平为(1.69±2.28)mm,双眼眼轴长度相关性差,相关系数为0.549,差异无统计学意义(P=0.816>0.05)。结论 A超分段测量法可对硅油填充眼进行准确可靠的生物测量。Objective To evaluate the accuracy of using A-scan echography in biomeasurement for silicone oil-filled eyes.Methods Twenty-nine silicone oil-filled eyes in 29 patients in our hospital from January 2008 to December 2008 had been prospectively chosen.All of these eyes underwent silicone oil removal.The axial length(AL)of all the silicone oil-filled eyes were measured with A-scan ultrasound and compared before and after surgery.A-scan echography of silicone oil-filled eyes were taken contact method at supine position.The silicone oil bubble’s length equalled 0.644 times the length measured with the sound velocity 1 532 m·s-1.The depth of the water layer beneath the oil bubble was also included in the calculation of the AL with echography.The formula was that AL of silicone oil-filled eyes equalled anterior chamber depth plus lens thickness plus 0.644 times the apparent length of silicone oil bubble plus the depth of water layer beneath the oil bubble.Results The average AL were(26.11±2.85)mm and(26.06±2.80)mm with A-scan echography before and after surgery,the difference at the same eye between before and after surgery was(0.06±0.27)mm.There was high correlation between before and after surgery(r=0.995),but no difference was found(P=0.2830.05).The difference of AL was(1.69±2.28)mm between the eye with disease after surgery and contralateral eye,the correlation was poor(r=0.549)and no statistical difference was found(P=0.8160.05).Conclusion A-scan echography is an accurate and reliable biomeasurement for silicone oil-filled eyes.
