摘 要:垂体分泌的卵泡刺激素(follicular stimulation hormone,FSH)自月经周期前几天[前一周期的黄体晚期,黄体生成素(LH)峰后12d]开始上升,在早卵泡期阶段(相对28d周期约一周时间)分泌达到最高水平即阈值,并维持一定时间段即窗口期,以促成那些已募集的从静息状态进入生长期的卵泡完成选择、卵泡优势发育过程。其中对FSH反应最敏感的卵泡(低FSH阈值)分泌雌激素增加,Pituitary secretion of follicular stimulation hormone (FSH) starts to rise a few days before the menstruation. In early follicle phase it reaches the highest level, and maintains in a certain period of time that is so-called FSH threshold window, in order to promote recruited follicles to grow and develop. According to the concept of FSH threshold which was firstly proposed by Brown, FSH concentrations below the threshold level may cause the failure of follicular development while FSH concentration above the threshold level can cause follicular development, and further increase in gonadotropin dose would lead to ovarian hyperstimulation syndrome (OHSS). Only the appropriate dose of gonadotropin can achieve the desired outcome of superovulation. Because many factors affect the ovarian response, the individual FSH threshold is variable, and a consistent predictor of ovarian response is still being explored. The initial dose of gonadotropin is rather individualized.
