老年多器官功能障碍综合征患者肠内外营养支持的比较

摘　　要：目的对比分析老年多器官功能障碍综合征(MODSE)患者肠内和肠外营养支持的效果。方法将60例老年MODS患者随机分为肠内营养支持组(EN组)和全肠外营养支持组(TPN组),每组30例,在摄入同等热量和氮量的条件下,检测两组患者营养支持前后血红蛋白(Hb)、血清白蛋白(ALB)、丙氨酸氨基转移酶(ALT)、肾功能、血糖、血电解质等指标,同时观察两组营养支持时并发症的发生情况、平均住ICU天数和营养支持费用等。结果 EN组营养支持后患者Hb和ALB水平明显高于TPN组,差异有统计学意义(P<0.05)。EN组共20例次发生消化道出血、腹泻和消化道二重感染;TPN组共66例次发生上述并发症,两组各项并发症发生率间差异有统计学意义(P<0.05)。平均每天营养支持费用EN组为(282.8±76.3)元,明显低于PN组的(517.6±82.6)元,差异有统计学意义(P<0.05)。平均住ICU的天数EN组为(6.08±3.94)d,明显短于PN组的(11.9±4.85)d,差异有统计学意义(P<0.05)。结论 MODSE患者及时应用营养支持能促进康复,且肠内营养有费用低、并发症少、实施方便等优点,更适于临床应用。Objective To compare the efficacies of enternal nutrition （EN） and total parenteral nutrition （TPN） in the management of multiple organ dysfunction syndrome in the elderly （MODSE）.Methods Totally 60 patients with MODSE were equally randomized into EN group and TPN group.Hemoglobin,serum albumin,alanine aminotransferase,renal function,blood glucose,and electrolytes were determined before and after nutritional supports.Complications,hospital stay,and cost for nutrition support were also analyzed.Results After nutritional supports,Hb and albumin levels were significantly higher in EN group than in TPN group （P〈0.05）.The frequency and severity of complications were significantly lower in EN group （P〈0.05）.The daily costs for nutritional supports were （282.8±76.3） yuan in EN group and （517.6±82.6） yuan in TPN group （P〈0.05）.The mean hospital stay was （6.08±3.94） days in EN group and （11.9±4.85） days in TPN group （P〈0.05）Conclusion Timely nutrition support can improve the rehabilitation of MODSE,decrease complications,and lower expenses.EN is superior to TPN in many aspects.
