深部脑白质缺血患者MRI表现与颈动脉粥样硬化斑块和颈动脉内膜中层厚度关系的研究

摘　　要：目的探讨颈动脉粥样硬化对深部脑白质缺血患者MRI表现和颈动脉内膜中层厚度(IMT)的影响以及影像学分级程度与内膜中层厚度的关系。方法选择MRI表现符合皮质下深部脑白质缺血诊断标准的患者30例,采用彩色超声诊断仪探测颈动脉IMT及粥样硬化斑块数目,并与MRI表现正常者30例(对照组)比较。结果深部脑白质缺血患者MRI表现程度与颈动脉IMT比较差异有统计学意义(P<0.01),两两比较,中度患者颈动脉IMT高于轻度,重度患者颈动脉IMT高于轻度与中度患者,差异均有统计学意义(P<0.01)。深部脑白质缺血患者斑块检出率(93%)与对照组斑块检出率(27%)比较,差异有统计学意义(P<0.01)。结论深部脑白质缺血患者颈动脉粥样硬化发生率高;深部脑白质缺血患者MRI表现程度不同,颈动脉IMT亦不同。Objective To investigate the impact of carotid atherosclerosis to the manifestations in magnet resonance imaging （MRI） and carotid intima-media thickness （IMT） in patients with ischemia of the deep white matter of brain,and the relationship between the extent of radiological grading and the IMT.Methods Totally 30 patients whose MRI manifestations were consistent with diagnostic criteria ischemia of deep subcortical white matter of brain were selected.The carotid IMT and the number of atherosclerotic plaques were detected by using color ultrasound diagnostic apparatus,and the results were compared with those 30 patients （control group） whose MRI manifestations were normal.Results The extent of radiological grading and carotid IMT in patients with deep white matter ischemia were significantly different from those in the control group （P〈0.01）.As comparing each two groups of different radiological grades,the carotid IMT of the moderate patients was higher than that of the mild patients,and the carotid IMT of severe patients was higher than those of the mild and moderate patients （P〈0.05）.The plaque detection rate （93%） in patients with deep white matter ischemia was higher than that （27%） in patients of the control group （P〈0.01）.Conclusion The morbidity of atherosclerosis in patients with deep white matter ischemia is high;the pathological extents of grading in MRI and carotid IMT are also different.
