摘 要:目的通过64层螺旋CT(VCT)扫描三维重建和多平面重建在环枢椎骨折和/或脱位中的应用,探讨三维影像(3D)和二维影像(2D)在诊断环枢椎损伤中的临床应用价值。方法对临床诊断有环枢椎骨折和/或环枢关节脱位的32例外伤患者使用VCT容积扫描,并在工作站进行三维重建(VR)和多平面重建(MPR),并对3D和2D图像进行对比。结果3D图像光滑、细腻、逼真,清楚显示骨折线走行方向、骨折范围、位置、类型、折块移位情况及与周围骨骼的解剖关系,对骨折显示立体直观,能清楚显示骨折、脱位的畸形表现等空间关系。2D影像对细小骨折显示较好,且可同时显示骨折部位的软组织及脊髓损伤情况。结论64层螺旋cT容积扫描可获得高质量3D图像,3D图像在空间信息方面明显优于2D图像,3D图像显示对临床处理有较大的指导作用。但2D图像是3D图像重建的基础,两者在临床应用中各有优缺点,两者互相结合是诊断环枢椎骨折及脱位的最佳方法。Objective To evaluate three dimensional( 3 D) image and two dimensional(2D) image in diagnosis of at- lantoaxial spine injuries through application of three - dimensional reconstruction and multi - planar reconstruction (MPR) with 64 spiral CT(VCT) scanning. Methods 32 cases with fracture or dislocation of Atlantoaxial vertebrae had been done 64 slices spiral CT scan and 3D reconstruction (VR) and Multi planar reconstruction (MPR). Results 3D images manifested smooth, detailed and really; clearly showed the courses, length position, type of fracture line, fragmental shift and neighboring -skeletal anatomic relationship; and also clearly showed stereoscopic image of fracture line, and spatial relationship of bone fracture and abnormalities. 2D image better showed the fine bone fracture and abutting soft tissues changes and spinal cord compression. Conclusion 64 Spiral CT volume scan can provide high quality of 3D image. 3D image is prior to 2D image in spatial resolution and clinical application. 2D image is the basic of 3D image reconstruction, both of 3D and 2D have their own advantage and shortage, accuracy of imaging manifestation can be improved when 3 D and 2D are used together.
