摘  要:目的探讨非小细胞肺癌CD44途径免疫逃逸的机制及其临床意义。方法采用免疫组化EnVison法,检测Fas、CD44s和CD44v6基因蛋白在非小细胞肺癌及癌旁正常肺组织中的表达,并分析其表达与肿瘤病理分期、分化程度和淋巴结转移的关系。结果在癌旁肺组织中Fas的表达阳性率为100%,在肺鳞癌和腺癌组织表达阳性率分别为62.5%和46.4%,在非小细胞肺癌组织中Fas的表达减弱(Hc=37.068,P〈0.01)。CD44s和CD44v6在癌旁正常肺组织中未见表达;CD44s在肺鳞癌和腺癌组织表达阳性率分别为84.4%和46.4%,CD44v6在肺鳞癌和腺癌组织表达阳性率分别为81.3%和39.3%。Fas、CD44s和CD44v6的表达均与肿瘤TNM分期、分化程度及淋巴结转移有关(u=2.476~3.986,Hc=10.657~20.210,P〈0.01)。结论非小细胞肺癌通过癌细胞表面CD44分子的构型由标准型向变异体改变,降低其Fas的表达,从而逃脱Fas-FasL介导的细胞凋亡。非小细胞肺癌Fas、CD44s和CD44v6基因蛋白的检测可作为肿瘤进展程度的判断指标;非小细胞肺癌的发病与CD44途径免疫逃逸相关。Objective To investigate the immune escape mechanism pathway of CD44 in non-small cell lung cancer(NSCLC) and its clinical significance. Methods The immunohistochemical EnVison method was used to detect expressions of Fas,CD44s and CD44v6 in NSCLC and adjacent normal lung tissue.The relationship between their expressions and pathological stage,tumor differentiation,lymph node metastasis was analyzed. Results The positive expression of Fas in adjacent lung tissue was 100%;in squamous cell carcinoma(SCC) and adenocarcinoma(AC) was 62.5% and 46.4%,respectively;in NSCLC,the expression of Fas decreased.CD44s and CD44v6 were not found in adjacent normal lung tissue.The expression rate of CD44s in SCC was 84.4% and in AC was 46.4%,while CD44v6 was 81.3% and 39.3% in SCC and AC,respectively.The expressions of Fas,CD44s and CD44v6 in tumors were related to TNM staging,differentiation and lymph node metastasis(u=2.476-3.986,Hc=10.657-20.210,P0.01). Conclusion Through the surface CD44 molecule conformation,NSCLC was transformed from a standard model to a variant,decreased the expression of Fas,escaping Fas-FasL-mediated apoptosis.The detection of Fas,CD44s,and CD44v6 can be used as markers to predict the process of the cancer.The occurrence of NSCLC correlates with CD44 pathway immune escape.
