摘  要:目的探讨64层CTA在血管痉挛性颅内动脉瘤影像学诊断价值。方法回顾分析2007年12月至2009年10月经手术证实并随访的12例颅内动脉瘤患者的临床资料,CTA检查时均有不同程度颅内血管痉挛。结合数字减影血管造影(DSA)检查结果,通过调整窗宽窗位,分别诊断血管痉挛程度及动脉瘤情况。结果12例患者中,轻度痉挛5例,中度痉挛3例,重度痉挛4例;单发动脉瘤10例,多发动脉瘤2例;动脉瘤分布:前交通动脉瘤7个,右侧大脑中动脉分叉处2个,左侧大脑中动脉分叉处1个,右侧后交通动脉瘤2个,左后交通动脉瘤2个。结论脑血管痉挛性颅内动脉瘤进行CTA成像,通过调窗及显影期相选择对诊断动脉瘤准确性较高。Objective To investigate the value of 64-slice CT Angiography(CTA) in the diagnosis of intracranial vasospastic aneurysms.Methods We reviewed 12 cases of intracranial aneurysms confirmed by surgery and followed-up in our hospital from Dec 2007 to Oct 2009.All of them had varying degrees of intracranial vascular spasm in CTA examination.combined with DSA results,by adjusting the window width and window level.The two associate chief physicians diagnosed of the degree of vascular spasm and whether there were aneurysms respectively.Results In 12 patients with aneurysms,mild angiospasm were found in 5 cases,moderate spasm in 3 cases,and severe spasm in 4 cases.Ten patients had single aneurysms,and multiple aneurysms were found in 2 cases,7 aneurysms distributed in the anterior communicating artery,2 in the right middle cerebral artery bifurcation,1 in the left middle cerebral artery bifurcation,2 in the right posterior communicating artery.The left posterior communicating artery had 2 aneurysms.ConclusionBy adjusting window width and window level,selecting the right phase,cerebral the CTA imaging in diagnosis of intracranial vasospastic aneurysms has a relative higher accuracy.
