摘  要:目的探讨单相抑郁症患者各脑叶CT值的改变与病程的关系。方法对不同病程的100例单相抑郁症患者进行颅脑CT扫描,测量各脑叶的CT值,并与100例正常人进行对照比较。结果 1.患者组的左额叶CT值(40.22±1.80)比正常对照组(41.37±1.25)明显减小,差异有显著性(P<0.01),而右额叶CT值无明显差异(P>0.05);左、右颞叶CT值(40.13±2.82)、(44.53±2.99)比正常对照组(41.25±2.59)、(45.38±2.82)明显减小,差异有显著性(P<0.01)。2.患者组左额叶CT值与年龄(Beta=-0.319、P<0.01)、病程(Beta=-0.355、P<0.01)成负相关,右额叶CT值与年龄(Beta=-0.218、P<0.01)成负相关,而与病程无显著相关性(P>0.05);余各脑叶未发现与病程等有相关性。结论与正常人相比,单相抑郁症患者左额叶(优势半球)、颞叶萎缩,且左额叶萎缩程度与病程有关,提示抑郁症或许是中枢神经系统的退行性疾病。Objective To explore the changes of CT value in cerebral lobes in patients with unipolar depressive disorder and its relation with duration. Methods Cerebral CT was performed in 100 patients with unipolar depressive disorder. CT value of all cerebral lobes were compared with the results of 100 normal subjects. Results 1. Left frontal CT value of the unipolar depression(40.22 ± 1.80) was significantly smaller than that of control group(41.37± 1.25), while there had no significant difference in right frontal index. 2. Left frontal CT value was significantly positively related to age( Beta = - 0. 319 ,P 〈 0.01 ) and duration of depression ( Beta = - 0.355 ,P 〈 0.01 ), and right frontal index was significantly positively related to age( Beta = -0.218 ,P 〈0.01 ) ,while had no gignificant correlation with duration of u- nipolar depression(P 〉 0.05). Conclusion Compared with normal subjects, the atrophy would be present in the left frontal( dominant semi -sphere) and temporal lobe of unipolar depression, and the degree of atroply may be associated with duration of depression. It may sugest that depreesion is a degenerative disease of central nervous system.
