星状神经节阻滞对乳腺癌围手术期细胞免疫和体液免疫的影响

摘　　要：[目的]观察星状神经节阻滞(SGB)对乳腺癌患者围术期细胞免疫及体液免疫的影响。[方法]择期乳腺癌改良根治术女性患者80例,随机分成对照组和干预组,观察干预前(T1)、干预后d1(T2)和干预后d5(T3)时CD3+、CD4+、CD8+、NK细胞活性以及IgG、IgA、IgM的变化。[结果]2组各时点CD3+、CD4+、CD8+均无显著变化(P﹥0.05)。Ⅰ组T2、T3时NK活性明显低于T1(P﹤0.05或P﹤0.01),Ⅱ组仅T2时NK活性降低(P﹤0.05),T3时NK活性恢复,明显高于Ⅰ组(P﹤0.05)。与T1比较,T2、T3时2组IgG、IgA、IgM均有不同程度下降(P﹤0.05,P﹤0.01),T3时Ⅱ组IgG明显高于Ⅰ组(P﹤0.05),T2、T3时Ⅱ组IgA明显高于Ⅰ组(P﹤0.05)。[结论]术前SGB可减轻乳腺癌改良根治术患者细胞免疫和体液免疫的抑制程度。[Objective] To observe the effects of stellate ganglion block（SGB）on cellular immunity and humoral immu-nity function in patients received modified radical mastectory（MRM）.[Methods] Eigty female patients were schedualed for selective MRM,who were randomly divided into two groups：control group and intervention group.Blood samples were taken after pre-intervention（T1）,intervention（T2）and 5 day after operation（T3）for determination of CD3^＋,CD4^＋,CD8^＋,NK activities and IgG,IgA,IgM.[Results] NK activities were significantly decreased atT2 and T3,as compared with baseline in groupⅠ,while which were significantly decreased only at T2.NK activities were much higher at T3 in groupⅡthan those in group Ⅰ.Compared with T1,IgG,IgA,IgM at T2 and T3 in groupⅡwere significantly higher than those in groupⅠ（P﹤0.05,P﹤ 0.01）.[Conclusion] SGB before operation can improve the inhibition degree of cellular and humoral immunologic function caused by stress.
