摘 要:目的探讨全氟化碳(perfluorocarbon,PFC)汽化吸入对急性肺损伤动物肺外脏器(小肠、肝、肾)MMP-9表达的影响。方法将30只新西兰兔随机分为常规通气组(CMV组)、呼气末正压组(PEEP组)、PFC汽化吸入组(PFC组)、PFC汽化吸入+呼气末正压组(PEEP+PFC组)、对照组(C组)。将兔气管切开,插管连接呼吸机进行机械通气,通过人工海水气管内注入制作ALI模型后,前4组动物按照不同的实验分组进行相应的通气治疗2h,调整为常规机械通气4h后处死,对照组撤除呼吸机。实验结束后取各组动物的小肠、肝、肾组织,应用免疫组织化学技术从蛋白水平观察MMP-9在肺外脏器(小肠、肝、肾)的表达。结果肺外脏器免疫组织化学评分比较显示,PEEP组和PFC+PEEP组肾组织中MMP-9蛋白的表达水平较CMV组下降(P<0.05)。CMV组和PFC组肾组织中MMP-9蛋白表达水平差异无统计学意义(P>0.05)。CMV组、PEEP组、PFC组和PFC+PEEP组小肠组织和肝组织中MMP-9蛋白的表达水平差异无统计学意义。结论本实验无法证实汽化吸入PFC能够通过减少MMP-9蛋白在肺外脏器(小肠、肝、肾)中的表达,从而减轻MMP-9对肺外脏器的损伤。Objective To investigate the effect of vaporized perfluorocarbon therapy on acute lung injury in animal models and to elucidate the mechanism of the therapy from the expression of matrix metalloproteinase - 9 ( MMP - 9 ) in extra - pulmonary organs. Methods Thirty female News Zealand rabbits were randomly divided into five groups: conventional mechanical ventilation group ( CMV), positive end expiratory pressure ventilation group( PEEP), vaporized - PFC group ( PFC), vaporized - PFC with PEEP group ( PFC + PEEP), and control group. Infusion of artificial sea water through tracheal intubation made ALl models. The animals in former four groups received different treatment for 2 h followed by conventional ventilation for 4 h. And the control group was weaned from ven- tilation. Then animals were sacrificed and small intestines,livers and kidneys in the same area were cut into small slides. The MMP - 9 protein expression in extra pulmonary organs (small intestine, liver, kidneys) was observed by immunohistoehemistry. Results The expression of MMP - 9 in kidneys of experimental animals exposed to PEEP and PFC + PEEP were significantly lower than those in CMV group by immunohistoehemistry. Conclusions Vaporized PFC does not inhibit the damage of extrapulmonary organs through down -regulating expression of MMP -9.
