摘  要:目的:应用全自动血细胞分析仪定量检测外周血中的网织红细胞血红蛋白含量(Ret-He),对其进行方法学评价,并研究Ret-He在缺铁性贫血诊断和鉴别诊断中的价值。方法:应用SYSMEX-XE2100全自动血细胞分析仪检测常用的红细胞参数及网织红细胞血红蛋白含量。评价该方法检测Ret-He的批内、批间变异系数、重复性和稳定性。分别检测40例正常健康人、40例缺铁性贫血患者、40例地中海贫血患者和40例肾病性贫血的红细胞参数(RBC、HGB、MCV、MCH、HFR、Ret-He)并进行组间比较。结果:批内变异系数及批间变异系数均<3%,冷藏保存48h内结果稳定。正常对照组、缺铁性贫血患者组、地中海贫血组和肾病性贫血组的Ret-He中位数分别为30.03、23.84、29.82和31.32pg;缺铁性贫血组Ret-He分布低于其余各组,差异具有统计学意义(P<0.001),其余各组Ret-He分布差异均无统计学意义。结论:Ret-He优于传统的血液学及生化指标,在缺铁性贫血的诊断和鉴别诊断中有一定的应用价值,且简便、快速、价廉,可广泛应用于缺铁性贫血的诊断和鉴别诊断。Objective To quantificationally detect the reticulocyte hemoglobin content (Ret-He) by automated blood cell analyzer, and to evaluate the application of Ret-He in the diagnosis and differential diagnosis of iron deficiency anemia. Methods The red blood cell indices (RBC, HGB, MCV, MCH, HFR, and Ret-He) and reticulocyte hemoglobin content were detected by SYSMEX-XE2100 automated blood cell analyzer in 40 healthy controls (control group), 40 patients with iron deficiency anemia (iron deficiency anemia group), 40 patients with thalassemia (thalassemia group), and 40 patients with renal anemia (renal anemia group). The within-run coefficient of variatron, between-run coefficient of variatron, repeatability, and stability of detecting Ret-He by this method were evaluated. Results Both of the between-run coefficient of variatron and within-run coefficient of variatron were less than 3%. The results of Ret-He detection of cryopreservation samples showed acceptable stability within 48 hours. The median of Ret-He in control group, iron deficiency anemia group, thalassemia group, and renal anemia group were 30.03, 23.84, 29.82, and 31.32 pg respectively. The distribution of Ret-He in iron deficiency anemia group was less than other groups (P 0.001), while there were no significant differences among the other three groups. Conclusion Ret-He is superior to conventional hematologic and biochemical indicators of the diagnosis and differential diagnosis in iron deficiency anemia. Analyzing Ret-He is a simple, rapid, and inexpensive way which should be widely used in the diagnosis and differential diagnosis in iron deficiency anemia.
