不同年龄段急性脑梗死患者颅外段颈动脉多普勒超声相关临床分析

摘　　要：目的评价不同年龄段急性脑梗死患者颅外段颈动脉多普勒超声及其相关临床资料分析。方法将急性脑梗死患者757例,根据年龄分为<60岁组(134例)、60～69岁组(140例)和≥70岁组(483例),分别对年龄、性别、疾病、再发脑梗死、死亡、心源性死亡及颈动脉超声进行比较。结果 3组患者大多合并有高血压;≥70岁组患者脑梗死、再发脑梗死及冠心病和心力衰竭的比例均明显高于<60岁组和60～69岁组;且前者首次及再梗死时死亡和心源性死亡的比例明显高于<60岁组和60～69岁组,再梗死的病死率明显高于首次梗死(P<0.05)。结论脑梗死患者颈动脉粥样硬化与年龄有关,随年龄的增长,发生的比例增加,尤其是年龄≥70岁患者。对老年患者及其合并冠心病者应重点防治。Objectives To assess clinical data and extracranial carotid Doppler ultrasonography in cases of different ages with acute cerebral infarction（ACI） in order to provide objective basis for preventing and treating CI. Methods 757 cases of ACI were divided into in 3 groups according to their ages：group Ⅰ included 134 cases aged 〈60 years,group Ⅱ included 140 cases aged between 60-69 years and group Ⅲ included 483 cases aged 970 years. Clinical data included ages,gender, diseases,recurrence of CI, mortality, cardiogenic death and conditions of carotid atherosclerosis （AS） were analyzed and compared between different groups. Results The rates of CI,re-infarction and concurrent coronary heart diseases and heart failure were significantly higher in group Ⅲ than those in other groups. The mortality of first infarction and re-infarction and cardiogenic death were much higher in group Ⅲ than those in other groups. The fatality of re-infarction was much higher than that of first CI （P 〈 0.05）. Conclusions Carotid AS exists in cases with CI which is related to age and increases gradually with age, especially in those aged ≥70 years. Attention should be paid to prevention and treatment of carotid AS in elder patients and those accompanied with coronary heart disease.
