踝臂指数与老年缺血性脑卒中的相关性研究

摘　　要：目的观察踝臂指数(ABI)与老年缺血性脑卒中的相关性。方法选择住院的老年缺血性脑卒中患者(脑卒中组)180例及非脑卒中老年患者(非脑卒中组)100例,行ABI测量。收集2组患者相关危险因素和实验室检查资料,进行分析。结果脑卒中组和非脑卒中组患者ABI异常患病率分别为37.2%和9.0%;与非脑卒中组比较,脑卒中组患者ABI明显下降(0.95±0.21vs1.06±0.14,P<0.01);脑卒中组患者血脂异常和高血压患病率明显高于非脑卒中组(P<0.05,P<0.01)。logistic回归分析显示,高血压(OR=1.824,95% CI:1.007～3.305,P<0.05)和ABI异常(OR=5.292,95% CI:2.477～11.309,P<0.01)与老年缺血性脑卒中明显相关。结论 ABI降低的老年患者缺血性脑卒中的发生增加,临床应重视老年患者ABI的检查。Objective To investigate the association of ankle-brachial index（ABI） with ischemic stroke in geriatric patients. Methods A cross-sectional study was conducted in 180 hospitalized geriatric-patients with ischemic stroke（ischemic stroke group） and 100 geriatric patients without stroke who served as the control（non-stroke group）. ABI was measured for all patients. Relevant risk factors and laboratory findings were also collected. Results The proportion of patients with abnormal ABI in the ischemic stroke group and the non-stroke group was 37.2% and 9.0% ,respectively,the level of ABI for the two groups was 0.95 ± 0.21 and 1.06 ± 0.14 , respectively. The differences between the 2 groups were statistically significant （P 〈 0.01）. The prevalence rates of dyslipidemia and hypertension were significantly higher in stroke group than in nonstroke group. Logistic regression analysis indicated that there was statistically significant correlation between geriatric ischemic stroke and hypertension （OR = 1. 824,95% CI. 1. 007-3. 305, P〈0. 05） or ABI （OR = 5.292,95% CI. 2.477-11.309, P〈 0.01）.Conclusion The occur- rence of ischemic stoke increases in geriatric patients with decreased ABI, therefore attention should be paid to ABI measurement in geriatric patient in clinical practice.
