摘 要:目的探讨CD4+T淋巴细胞内三磷腺苷(adenosine triphosphate,ATP)含量与原发性肝癌(肝癌)肝移植术后肿瘤复发的关系。方法 64例次肝癌肝移植,56名健康志愿者(对照组)纳入该研究。采集外周血标本,用ImmuKnowTM免疫细胞功能测定试剂盒检测CD4+T淋巴细胞内ATP值。根据样本采集时肝癌肝移植患者是否发生肝癌复发,将采集的64份全血标本分为非复发组(51份)和复发组(13份)。结果健康对照组、非复发组、复发组的CD4+T淋巴细胞内ATP含量分别为(459±121)ng/ml、(308±101)ng/ml、(138±97)ng/ml,复发组的ATP值明显低于非复发组和对照组。ATP值降低(<147ng/ml)时肝癌复发的风险明显增加,与肝癌复发存在良好的相关性。结论 CD4+T淋巴细胞内ATP含量与肝癌肝移植术后肿瘤复发具有良好相关性,监测肝癌肝移植术后患者的上述指标对评估其免疫状态、制定个体化治疗方案具有实用价值。Objective To explore the relationship between the adenosine triphosphate (ATP) level in CD4 ^+ T lymphocyte and the recurrence of hepatocellular carcinoma after liver transplantation. Methods ImmuKnowTM test kits were used to measure the ATP level in CD4 ^+ T lymphocytes for immune cells function evaluation. Immuknow testing was performed in 64 cases who underwent orthotopic liver transplantation for hepatocellular carcinoma (HCC) and 56 healthy people as the control group. The patients were further divided into no recurrence group (n = 51 ) and recurrence group (n = 13 ). Results The ATP level within CD4 ^+ T lymphocyte in the control group, no recurrence group and recurrence group were (459 ± 121 ) ng/ml, ( 308 ± 101 ) ng/ml, (138 ±97) ng/ml, respectively. The ATP level in recurrence group was significantly lower than that in the control group and no recurrence group. The lower ATP level ( 〈 147 ng/ml) showed good correlation with recurrence of hepatocellular carcinoma after liver transplantation. Conclusion The ATP level within CD4 ^+ T lymphocyte has good correlation with the recurrence of hepatocellular carcinoma after liver transplantation. The regular measurement of ATP level in CD4 ^+ T lymphocytes is useful for post-operation immune function monitoring and individual immunosuppression therapy in patients who undergo liver transplantation for HCC.
