摘 要:目的探讨Ho∶YAG激光在消化系统疾病内镜治疗中的应用价值。方法采用Ho∶YAG激光对新西兰白兔的胃和结肠组织进行在体照射,观察其汽化切割和止血特性及组织损伤病理特点,用计算机图像分析系统精确测量其热损伤带宽度,并通过重量丢失实验检测其可控性。结果Ho∶YAG激光具有精细的组织切割和可靠的止血特性,其热损伤带宽度为400~800μm,且不受能量脉冲设置、照射时间、照射方式及液体介质等照射条件的影响,组织重量丢失与激光总能量之间呈良好的线性关系,具有良好的可控性。结论Ho∶YAG激光是用于消化系统疾病内镜治疗的安全、有效的理想激光。Objective To evaluate the endoscopic use of Ho∶YAG laser in the treatment of gastrointestinal diseases. Methods Tissue effects of a Ho∶YAG laser operating at a wavelength of 2.1 μm with a maximum power of 15 W and 10 different energy pulse settings were systemically evaluated on stomach and colon in vivo in the New Zealand rabbits. Microscopic examination, computer image analysis and mass loss experiments were performed. Results Microscopic examination and computer image analysis of the irradiated tissues revealed that zones of thermal damage extended 400-800 μm from ablation sites, which is not affected by different exposure conditions including energy pulse settings, exposure time, irradiation distance and fluid media. Mass loss experiments showed that the tissues mass loss increased linearly with delivered radiant exposure. Conclusions Our findings indicate that Ho∶YAG laser produces less thermal damage in the surrounding tissues than current widely used endoscopic laser systems such as continuous wave Nd∶YAG laser, and the ablation rate can be precisely controlled, and the Ho∶YAG laser may become a promising alternative means for endoscopic laser therapy of digestive diseases.
