摘 要:目的探讨COX-2及VEGF-C在人上尿路尿路上皮癌中的表达及其临床意义。方法采用免疫组化技术二步法分别检测43例上尿路尿路上皮癌和12例正常对照组织中COX-2和VEGF-C的表达,并分析其表达水平与患者各临床病理参数之间的关系,以及上尿路尿路上皮癌中COX-2与VEGF-C二者之间表达的相关性。结果 COX-2与VEGF-C的表达主要位于上尿路尿路上皮癌细胞的胞质中,表达阳性率分别为76.7%及67.4%,均高于各自在正常对照组织中的表达水平(P<0.01)。COX-2和VEGF-C的表达程度与病理分级、临床分期相关(P均<0.05)。COX-2在有、无淋巴结转移的上尿路尿路上皮癌中的表达差异无统计学意义(P=0.09),VEGF-C在有、无淋巴结转移的上尿路尿路上皮癌中的表达差异有统计学意义(P=0.011)。COX-2和VEGF-C在上尿路尿路上皮癌中的表达具有正相关性(r=0.675,P<0.01)。结论 COX-2和VEGF-C的表达与上尿路尿路上皮癌的病理分级、临床分期相关。VEGF-C还与肿瘤淋巴结转移相关。COX-2和VEGF-C在上尿路尿路上皮癌中的共同表达提示二者可能在上尿路尿路上皮癌的发生、发展过程中起协同作用。Objective To assess the clinical significance and expression of cyclooxygenase-2 (COX-2) and vascular endothelial growth factor C (VEGF-C) in human upper urinary tract urothelial carcinoma(UUTUC).Methods Two-steps immunohistochemistry was applied to detect the expression pattern of COX-2 and VEGF-C in 43 UUTUC tissues and 12 normal upper urinary tract(UUT) tissues.The possible relationship of COX-2 and VEGF-C with tumor grade,stage,lymph node metastasis was evaluated.Results The expression of COX-2 and VEGF-C were mainly distributed in tumor cell cytoplasm,positive expressions were found in 76.7% (COX-2) and 67.4% (VEGF-C) patients with UUTUC respectively,both positive expression rates in UUTUC were significantly higher than that 12 normal UUT tissues (P0.01).Both the expression of COX-2 and VEGF-C in tumor cells correlated with tumor grade and stage of disease (P0.05).The expression level of VEGF-C correlated with lymph node metastasis(P=0.011),while the correlation was not found in COX-2(P=0.09).VEGF-C expression had positive correlation with COX-2 expression in UUTUC tissues (r=0.675,P0.01).Conclusions Both COX-2 and VEGF-C expressions are related to the grade and stage in UUTUC.VEGF-C has relationship with lymph node metastasis inUUTUC.The co-expression pattern of COX-2 and VEGF-C in UUTUC suggest that they may act synergistically in development and progression in UUTUC.
