腹腔镜结肠癌手术体位的探讨

摘　　要：目的:探讨改良手术体位对腹腔镜结肠癌手术术野暴露、肢体受压、稳定性及对术者和洗手护士操作的影响。方法:选择行腹腔镜结肠癌手术病人104例,随机分成改良组和传统组各52例。传统组采用传统的手术体位:仰卧头低10～15度,向左或右侧15～20度;改良组采用在传统体位的基础上把双下肢水平分开90～100度,比较两组在体位固定的稳定性、肢体受压情况及对术者和洗手护士操作的影响情况。结果:"人"字形手术体位在体位固定的稳定性、肢体受压情况及对术者和洗手护士操作的影响有统计学意义(P<0.05)。结论:"人"形手术体位应用于腹腔镜结肠癌手术有利于暴露术野、提高稳定性,减少并发症、方便操作,提高了手术效率。Objective：To discuss the status of field exposure,physical pressure,body stability and convenience for surgeon and nurse when applying the improved surgical position in laparoscopy assisted colon cancer surgery.Methods：One hundred and four patients that underwent laparoscopy assisted colon cancer surgery were randomly divided into improved position group（52 cases）and conventional position group（52 cases）.Both groups were placed supine with their head 10 ° 15 ° lower,and 15 ° 20 ° to the left or right,while the improved position group patients were also required to separate their lower limbs to 90 ° 100 °,in the shape of the Chinese character of ＂Ren＂.Then the status of field exposure,physical pressure,body stability and convenience for surgeon and nurse of the two groups were compared with each other.Results：The above mentioned status of the two groups were significant different from each other（P〈0.05）.Conclusions：The improved ＂Ren＂ position is better for field exposure,physical pressure,and body stability;and is convenient for the surgeon and nurse,it can help to reduce complications and raise the surgery efficiency.
