消瘤汤联合腹腔热灌注化疗治疗进展期大肠癌术后的临床研究

摘　　要：目的:观察消瘤汤联合腹腔热灌注化疗治疗进展期大肠癌术后的疗效及其防治大肠癌术后复发的作用。方法:104例行手术治疗的进展期大肠癌患者,随机分为两组。治疗组54例,采用腹腔热灌注化疗同时口服消瘤汤,对照组50例,单用腹腔热灌注化疗方案进行治疗,观察两组生存质量、化疗药物不良反应、免疫功能的变化等。结果:治疗组生存质量的改善明显优于对照组(P<0.05);治疗组不良反应(白细胞减少、血小板减少、恶心呕吐、腹泻、神经毒性等)与对照组比较有显著性差异(P<0.05)。治疗组自然杀伤细胞活性(NK)、CD+4/CD8+水平均较对照组提高(P<0.05)。两组术前癌组织已浸透浆膜或术前已有局部淋巴结转移者1年及3年的生存率组间比较差异有显著性意义(P<0.05)。结论:消瘤汤联合腹腔热灌注化疗不但能减轻化疗药物毒副反应,而且能改善NK水平,提高CD4+/CD8+比值,增强巨噬细胞的吞噬功能,改善患者生活质量,对进展期大肠癌患者术后复发有一定的防治作用,并能提高患者的生存率。Objective：To observe the treatment and prevention efficacy on postoperative stage of advanced colorectal cancer operation treated by XiaoLiu decoction combined with intraperitoneal hyperthermic perfusion chemotherapy. Methods ：104 cases after advanced colorectal cancer operation were randomly divided into two groups： the treatment group with 54 patients, accepting intraperitoneal hyperthermic perfusion chemotherapy and Xiaoliu decoction orally, and the control group with 50 patients accepting intraperitoneal hyperthennic peffusion chemotherapy only. The quality of life, adverse drug reactions of chemotherapy and changes in immune function of two groups were observed. Results：there was significant difference on adverse reaction of treatment group （neutropenia, thrombocytopenia, nausea, vomiting, diarrhea, neurotoxicity, etc） compared with the control group （P 〈0.05 ）and the improvement of life quality of treatment group were better than the control group （P 〈0.05 ）. The natural killer cell activity （NK）, CD4^＋/CD8^＋ levels of treatment group increased than the control group （P 〈0.05）. The difference survival rate of patients with preoperative serous carcinoma or regional lymph node metastasis before surgery in 1-year and 3-year was significant between two groups （P 〈0.05）. Conclusion： The XiaoLiu decoction combined with intraperitoneal hyperthermic perfusion chemotherapy can effectively reduce adverse reactions while enhance NK cell activity, CD4^＋/CD8^＋ ratio and macrophage phagocytosis. This treatment can increase the survival rate and life quality of patients with advanced colorectal cancer operation while have preventive effect of recurrence.
