摘　　要：目的讨论以髓外浸润为首发表现的白血病的临床特点,提高对这一类白血病的认识。方法分析3例以髓外浸润为首发表现的白血病患者的临床表现、实验室及影像学检查,并进行文献复习。结果 3例白血病患者在确诊前均分别出现不同的髓外浸润症状:头痛、呕吐及颅骨破坏表现;会阴部感觉障碍,小便失禁;肢体麻木、疼痛。3例患者均经骨髓细胞形态学检查确诊为白血病,经抗白血病治疗,上述浸润症状得到明显改善。结论以颅骨破坏、颅高压、脊髓压迫症等为首发表现的白血病罕见,易给早期诊断带来困难,需提高对这一类白血病的警惕。Objective To study the clinical feature of leukemia with extramedullary infiltration as initial manifestation to recognize this kind of leukemia. Methods The clinical manifestations, laboratory and imaging studies of 3 leukemia patients with extramedullary infiltration as initial manifestation were analyzed and related literatures reviewed. Results Before final diag- nosis, 3 leukemia patients presented with different infiltrations： headache, vomiting, skull damage performance, or pcrineal sensory dysfunction, urinary incontinence, or limb numbness, pain. The 3 patients were all diagnosed as leukemia by cytological examination of bone marrow, and the infiltration symptoms improved after anto - leukemia treatment. Conclusion Leukemia with skull damage, intracranial hypertension and spinal cord compression as initial symptoms is rare and easy to create difficulties for early diagnosis. More attention should be paid to this type of leukemia.
