CEA蛋白、CEAmRNA的检测在大肠癌、胃癌早期诊断中的应用价值

摘　　要：目的研究CEA蛋白、CEAmRNA在大肠癌、胃癌患者血清、癌组织中的表达,为大肠癌、胃癌的早期诊断提供理论依据。方法选取21例大肠腺癌、20例胃腺癌住院患者和40例胃肠道良性疾病患者,检测其术前血清CEA蛋白表达;用免疫组织化学法和实时荧光定量RT-PCR法检测癌组织、癌旁组织及良性胃肠道疾病组织中CEA蛋白和CEAmRNA表达。结果大肠癌、胃癌癌组织CEA蛋白及CEAmRNA的表达均高于癌旁组织及良性胃肠道疾病组织(P<0.01,P<0.05),胃癌组织CEAmRNA表达与血清及组织中CEA表达比较有统计学显著差异(P<0.05);大肠癌组织CEAmRNA表达与血清CEA表达比较有统计学差异(P<0.05)。CEA和CEAmRNA在癌旁组织和良性胃肠道组织中表达率低。结论应用实时荧光定量RT-PCR法检测组织CEAmRNA在大肠癌和胃癌早期诊断中有重要价值。Objective To explore the epression of CEA, CEA mRNA in colorectal cancer, gastric carcinoma tissues, In order to provide the theory of early diagnosis of colorectal cancer and gastric carcinoma. Methods The serum CEA levels were estimated in 21 patients with colorectal cancer,20 patients with gastric carcinoma and 40 patients with optimum gastrointestinal disease before being operated. And expression rates of CEA, CEA mRNA in carcinoma tissues, adjacent tissues of carcinoma and optimum gastrointestinal disease tissues were estimated by immunohistochemical method and real time fluorescent quantitative RT- PCR method. Results The Expression rates of CEA and CEA mRNA in colorectal cancer and gastric carcinoma tissues were higher than that of adjacent tissues of carcinoma and optimum gastrointestinal disease tissues（ P 〈 0.01, P 〈 0.05 ）. The Expression rates of CEA mRNA in gastric carcinoma tissues differed statisticly from which of CEA in serum（ P 〈 0.05 ）. And the expression rates of CEA and CEA mRNA in adjacent tissues of carcinoma and optimum gastrointestinal disease were low. Conclusion The utilizing value of CEA mRNA detection in early diagnosis of eolorectal cancer and gastric carcinoma was great.
