乳腺癌Ki一67P—gp表达创面脱落细胞检查临床意义相关性研究

摘　　要：目的研究不同病期乳腺癌中Ki一67、P—gp表达及根治术中创面冲洗液脱落细胞情况，揭示其三者之间的相关性及与预后的关系。方法采用免疫组化方法检测90例不同病期乳腺癌患者手术根治标本中Ki一67、P—gP表达水平，采用离心沉淀法检测其根治手术创面冲洗液中有无癌细胞，同时对上述病例跟踪随访3年，观察其有无复发转移情况及3年生存率，计算Pearson相关系数。结果Ki一67在乳腺癌Ⅰ、Ⅱ、Ⅲ期的阳性表达率分别是60．0％，71．8％，90．5％；P—gp阳性表达率分别为36．7％，38．5％，57．1％；脱落细胞的阳性率分别是3．3％，7．7％，14．3％。结论Ki一67、P—gp、创面脱落细胞在乳腺癌Ⅰ、Ⅱ、Ⅲ期阳性表达率存在正相关关系。Objective To explore the expression of Ki - 67, P - gp and the exfoliated cells＇ of the wound, in order to clarify the relationships among the three and the prognosis. Methods The expressions of Ki - 67 and P - gp by the immunohistochemical were detected in 90 patients. Cancer ceils in the washingfluid of the wound surface were detected by centrifuge method. These patients were followed up for 3 years to see whether there were cancer recurrence and transfer. Meanwhile we also observed the 3 years survival rate. Finally we calculated the coefficient of correlation. Results The expression rate of Ki - 67 in breast cancer＇s stage Ⅰ ,stage Ⅱ , stageⅢ were 60. 0% ,71.8% ,90. 5%. The expression rate of P - gp in these three stages were 36. 7% ,38.5% and 57.1%. Cancer cells were found in 3.3% ,7. 7% and 14. 3% of the patients＇ washingfluid in the three stages. Conclusions Ki - 67, P - gp and the exfoliated cells have positive correlations to each other in the breast cancer of Ⅰ , Ⅱ and Ⅲ stage.
