112例不同消化道重建方式对患者生活质量及情绪影响分析

摘　　要：目的探讨不同消化道重建方式对患者生活质量及情绪的影响。方法选取2005年8月～2009年8月于笔者所在医院进行治疗的112例消化道肿瘤切除术患者为研究对象,将其随机分为Orr式Roux-en-Y食管空肠吻合术组(A组)36例、P式食管空肠Roux-en-Y吻合术组(B组)36例和间置横结肠代胃术(C组)40例,后将三组患者的并发症发生率进行统计,并于1、6个月将患者采用SF-36生活质量评价量表、汉密顿抑郁量表及汉密顿焦虑量表进行评价,后加以总结分析及比较。结果经研究比较发现,B组及C组并发症发生率明显低于A组,P均<0.05;三组患者1个月时的SF-36生活质量评价量表、汉密顿抑郁量表及汉密顿焦虑量表评分比较,差异无统计学意义;但是6个月时C组明显优于其他两组,均有显著性差异。结论间置横结肠代胃术在消化道重建术中是效果较好的术式,优点多,对患者的不良影响少,因此值得临床进一步研究及应用。Objective To study the influence of different digestive tract reconstruction for the quality of life and emotion of patients. Methods 112 cases patients who were treated with gastrointestinal tumor resection in our hospital from August 2005 to August 2009 were selected as research objective,and they were randomly divided into Orr-type Roux-en -Y esophagojejunostomy group（ A group）36 cases, P- type Roux- en- Y esophagojejunostomy group（ B group）36 cases and jejunal interposition pouch group（ C group）40 cases, then the incidence of complications and the SF- 36 scale, hamilton rating scale for depression and hamilton rating scale for anxiety at the first and sixth month were tested, and the results were compared. Results The incidence of complications of B group and C group were lower than A group （ P 〈 0.05 ）. SF - 36 scale, hamilton rating scale for depression and hamilton rating scale for anxiety of the three groups at the first month had no differences（ P 〉 0.05 ）, there were no statistical significance, the SF- 36 scale, hamilton rating scale for depression and hamilton rating scale for anxiety of the C group at the sixth month was better than that of the other groups（P 〈 0.05）, there were statistical significances. Conclusion The jejunal interposition pouch is the better one in the digestive tract reconstructions,there is more advantages and less influence for the patients, and it is worthy further study and using.
