后房型屈光性晶状体植入矫正高度近视

摘　　要：目的:评估后房型屈光性晶状体植入术矫正高度近视的有效性和安全性。方法:对14例高度近视患者(26眼)植入后房型屈光性晶状体(复视伦,杭州百康公司),随访12个月,观察其术后裸眼视力,最佳矫正视力,眼压,前房深度,角膜内皮细胞计数。结果:患者术前最佳矫正远视等值球镜度数为-16.51±6.34D;散光度为-1.62±0.86D;术后6个月平均裸眼远视力0.88±0.26,植入人工晶状体度数为-5.5^-18.5D,平均为-12.34±3.97D,术后12个月平均裸眼远视力0.83±0.26,最佳矫正远视力0.99±0.21。术后12个月裸眼远视力较术前最佳矫正远视力提高1行5眼,提高2行6眼,提高3行3眼;术后最佳矫正远视力较术前最佳矫正远视力提高1行4眼,提高2行5眼,提高3行8眼,提高4行2眼,所有术眼术后最佳矫正远视力不低于术前。术后6个月96.15%术眼(25/26)等值球镜在±1.00D内,92.30%术眼(24/26)等值球镜在±0.50D内;术后12月100%术眼(26/26)等值球镜在±1.00D内,92.30%术眼(24/26)等值球镜在±0.50D内。术前眼压平均15.95±3.72mm-Hg,术后6个月平均16.42±2.83mmHg,术后12个月平均15.90±2.21mmHg,差异无统计学差异(P>0.05)。术前角膜内皮细胞计数平均2 902.30±337.15个/mm2,术后6个月平均2798.45±334.54个/mm2,术后12个月平均2 759.30±265.01个/mm2,无显著差异(P>0.05)。前房深度术前平均3.68±0.30mm;术后6个月3.58±0.28mm,术后12个月3.55±0.27mm,差异显著(P<0.01)。结论:百康后房型屈光晶状体可以有效且安全地矫正高度近视,但远期效果需待进一步观察。Objective：To investigate the efficacy and safety of the implantation of posterior chamber phakic refractive lens for high myopia.Methods： In this study,26 eyes of 14 patients aged of 34.65±10.16 years with average spherical equivalent of-16.51±6.34 diopters（D）（range-5.5 to-18.5D） were implanted with BaiKang PCPIOL.Results： All of 26 eyes were implanted successfully and have been followed up for 12m（months）.uncorrected visual acuity（UCVA） was significantly improved,5eyes gained 1 line as compared to their preoperative.best spectacle-corrected visual acuity（BSCVA） 6 eyes gained 2 lines and 3 eye gained 3 lines.The postoperative best corrected visual acuity（BCVA） of every eye was better than the preoperative.There were no significant differences between pre-and 12m post-operatively mean corneal astigmatism（P=0.0434） and mean intraocular pressure（IOP）（P=0.9204）.The mean counts of endothelial cells was（2902.30±337.15）/mm2 preoperatively and（2759.30±265.01）/mm2 12m post-operatively.There was no significant difference（P=0.1082）.No severe complications occurred.Conclusions： The implantation of posterior chamber phakic refractive lens for high myopia is predictable,reversible and controllable with simple manipulation.No severe complications occurred in 12m post-operatively,and long-time follow-up is still required.
