急性脑梗死超早期静脉溶栓治疗的临床疗效分析

摘　　要：目的:分析急性脑梗死超早期静脉溶栓治疗的临床疗效。方法:对急性脑梗死超早期患者进行尿激酶静脉溶栓治疗46例,对照组44例。分别于治疗前和治疗后24 h、2周后进行神经功能缺损程度评分及脑CT复查,并记录溶栓组的不良反应。结果:静脉溶栓治疗后24 h、2周与治疗前比较差异均有统计学意义,对照组治疗后24 h、2周与治疗前比较,差别无统计学意义,治疗后24 h、2周静脉溶栓组与对照组相比,差别有统计学意义。静脉溶栓治疗后24 h内颅内出血、颅外出血、神经功能缺损加重的病例分别占2.2%、6.5%、6.5%。结论:超早期尿激酶静脉溶栓是治疗急性脑梗死的有效方法,不良反应率低,适用于在基层医院推广应用。Objective：To analyze the clinical efficacy of the ultra-early venous thrombolytic therapy of acute cerebral infarction.Methods： We treated 46 patients with super early acute cerebral infarction with intravenous thrombolytic therapy by urokinase,the result of which was compared with that of 44 cases in control group.Before and after the treatment in 24 hours and after two weeks,we gave a mark to the degree of neurological deficit and rechecked brain CT respectively,and recorded the adverse reactions of the thrombolysis group.Results： In the thrombolysis group,the changes after and before the treatment were compared,the difference possessed the statistical significance.In the control group,the changes after and before the treatment were compared too,the difference did not have significance.But the effects of the thrombolysis group was compared with that of the control one,the change had the statistical significance.After the treatment in 24 hours,the proportion of the intracranial hemorrhage,the extracranial hemorrhage and the aggravation of neurological deficit were 2.2%,6.5% and 6.5% respectively.Conclusion： The ultra-early venous thrombolytic therapy by urokinase is an effective way to treat the acute cerebral Infarction,and the adverse reactions is few,we should extend it at the grass-roots hospitals.
