应用MDCT曲面重建评价进展期直肠癌新辅助化疗疗效

摘　　要：目的通过对比进展期直肠癌新辅助化疗前后的多排螺旋CT曲面重建(CPR)影像,探讨其在进展期直肠癌新辅助化疗疗效评价上的应用价值。方法本组77例患者均为经手术治疗的进展期直肠癌患者。全部患者术前均应用FOLFOX-6化疗方案行新辅助化疗2周期。在新辅助化疗前后,分别对癌肿进行多排螺旋CT(MDCT)检查,并在影像学工作站上对癌肿进行沿病变长轴的CPR重建。统计依据沿病变长轴的CPR影像评价进展期直肠癌新辅助化疗疗效的准确度,并与传统的CT横断面影像学评价进行对比。统计在沿病变长轴的CPR影像上,依据癌肿长径评价进展期直肠癌新辅助化疗疗效与癌肿厚度评价间的差异有无统计学意义。结果依据沿病变长轴的CPR影像评价进展期直肠癌新辅助化疗疗效的准确度为83.11%,与传统的CT横断面影像学评价未见明显差异(P>0.05);在沿病变长轴的CPR影像上,依据癌肿长径评价进展期直肠癌新辅助化疗疗效与管壁厚度评价间,具有明显一致性(P<0.01)。结论 CPR重建可直观地显示新辅助化疗前后进展期直肠癌的长径与厚度变化,在进展期直肠癌新辅助化疗疗效的评价上,具有突出价值。Objective To explore the clinical value of multi-detector CT （MDCT） curved planar reformations （CPR） in evaluation of neoadjuvant chemothearapy effects of advanced rectum carcinoma. Methods MDCT scans were used to acquire images before and after two courses of FOLFOX-6 neoadjuvant chemothearapy in 77 patients with advan＇Ce rectum carcinoma. CPR technique was used to reconstruct the images along the long axis of rectum carcinoma on the workstation. The accuracy percentage in evaluation of neoadjuvant chemothearapy effects of advanced rectum carcinoma was calculated and compared between by CPR and by cross section. In CPR images, the appraising accuracy was also compared between by neoplastic length and by its thickness. Results The evaluation accuracy of neoadjuvant chemothearapy effects was 83.11% by CPR and there weren＇t significantly statistical difference between the two methods （P〉0.05）. In CPR images, the it＇s evaluation accuracy was coincident both by neoplastic length and by its thickness. （P〈0.01）. Conclusion CPR can directly display the changes both in the length of neoplastic and in its thickness after neoadjuvant chemothearapy, and it has satisfactory application in evaluation of neoadjuvant chemothearapy effects in advanced rectum carcinoma.
