直肠癌术后复发的CT诊断

摘　　要：目的探讨直肠癌术后局部复发的CT表现特点。方法回顾性分析经再次手术和病理证实的25例直肠癌术后局部复发患者的临床表现和CT征象。结果本组25例直肠癌术后复发病例造瘘术后复发CT表现为会阴或骶前区肿块,呈结节状或分叶状,骶前脂肪间隙消失,无造瘘术后复发CT表现为直肠壁增厚伴不规则偏心性肿块,管腔狭窄或闭塞,直肠周围脂肪间隙变狭窄移位。3例发生远处转移。结论 CT检查在直肠癌术后局部复发的早期诊断中具有重要临床价值。Objective To explore the clinical and CT features of local recurrence following resection of rectal cancer.Methods 25 cases with clinical and CT findings suffering from postoperative local recurence proved by the second look surgery and biopsy were analysized retrospectively.Results 25 cases with recurrent rectal cancer,CT scan showed nodular or lobulated mass in the areas of perineum and presacralis,as wall as with loss of presacral fat space,for patients without colostomy.CT finding included thickened rectal wall with unregular proximal tumor,rectal obliteration,rectostenosis and displacement of perirectal fat space.About 3 cases of them were accompanied with distant metastatic disease.Conclusion CT scanning may be helpful for early surveying local recurrence follwing resection of rectal cancer.
