滑膜肉瘤的超声诊断价值及其影像学比较

摘　　要：目的探讨滑膜肉瘤的超声诊断价值并与其他影像学方法进行比较。方法回顾性分析经病理证实的7例滑膜肉瘤患者的超声及其他各种影像学资料(包括X线、CT、MRI等)。结果 7例滑膜肉瘤均位于四肢关节旁,肿瘤表现为结节状、类圆形或分叶状肿块,超声显示肿瘤边界清楚、均匀实性低回声或边界不清、回声复杂(包括低回声区、不均匀稍强回声区等),彩超显示瘤体供血丰富。X线显示软组织肿胀、钙化、骨质侵蚀等间接征象,CT显示为稍低密度的不均匀软组织肿块,MRI在T1WI及T2WI都显示为不均匀信号。结论超声能较清楚的显示滑膜肉瘤的形态、大小及与邻近组织的关系,综合分析多种影像学检查能够提高诊断的准确性。Objective To explore the value of ultrasonography in the diagnosis of synovial sarcoma and comparison with other imaging modalities.Methods 7 patients with synovial sarcoma were retrospectively reviewed,all patients had previously undergone ultrasound and other imaging modalities,including X-ray,computed tomography,magnetic resonance imaging.Results All synovial sarcomas revealed a nodular,round or lobulated mass,which located in the extremities close to joints.Ultrasound demonstrated homogeneous hypoechoic areas with well-defined margins or heterogeneous more echoic areas with ill-defined margins.CDFI（color doppler flow imaging） demonstrated vascularity in the tumor.X ray showed indirect signs including soft-tissue swelling,calcifications and bone erosions.CT showed a heterogeneous soft-tissue mass with hypoattenuation.The lesions were usually inhomogeneous signal intensity on T1 or T2-weighted images.Conclusion Ultrasound provide useful information on the morphous,size,and relationship with surrounding tissues of synovial sarcoma and analysis of multiple imaging modalities are helpful for the diagnosis of synovial sarcoma.
