膀胱肿瘤合并前列腺增生的临床研究

摘　　要：目的 探讨同期经尿道膀胱肿瘤及前列腺电切的有效性及安全性.方法 选择我院自2003年6月至2009年2月住院治疗的167例合并膀胱肿瘤合并前列腺增生（BPH）的膀胱肿瘤患者,实验组（n=108）同期行膀胱肿瘤及前列腺电切,对照组（n=59）仅行膀胱肿瘤电切.两组在年龄、肿瘤大小、肿瘤部位、肿瘤分期分级、IPSS评分等方面差异无统计学意义（P〉0.05）.结果 所有手术顺利,术中未出现明显膀胱穿孔 实验组术后1个月内尿路感染率（6.5%）明显低于对照组（35.6%）,P〈0.01 两组肿瘤复发率分别为29.6%和40.7%,实验组低于对照组（P〈0.05） 两组复发时间分别为23.2和18.5个月,实验组长于对照组（P〈0.05） 前列腺窝种植例数分别为3例和2例,两组前列腺窝种植率差异无统计学意义（P〉0.05）.结论 对于合并BPH的膀胱肿瘤患者,同期行膀胱肿瘤及前列腺是比较安全可靠的方法,可以降低术后尿路感染发生率及肿瘤复发率,并不增加前列腺窝肿瘤种植.Objective To evaluate the efficacy and safety of transurethral bladder tumor and prostate resection simultaneously. Methods A total 167 cases of bladdr tumor complicated with benign prostate hyperplasia （ BPH） were treated from June 2003 to February 2009 in our hospital. The patients in test group （n=108） underwent TURBt and TURP at the same time, and those in control group received TURBt only. There was no significant difference between two groups in age, tumor size, tumor location, tumor stage and grade, internationl prostate symtoms score （IPSS）. Results All operations were preformed successfully, and no bladder perforation happened. One month postoperation urinary infection rate in test group （6.5% ） was lower than that in control group （35. 6% ） ,P 〈0. 01. The tumor recurrence rate in test and control groups was 29. 6% and 40. 7% respectively,P 〈0. 05. The recurrence time in test and control groups was 23. 2 months and 18. 5 months respectively ,P 〈0. 05. There were 3 and 2 cases of prostate seeding in test group and control group, respectively,P〉0.05. Conclusion Transurethral bladder tumor and prostate resection simultaneously is safe and effective for patients with bladder tumor complicated with BPH. It can reduce postoperative urinary infection rate and tumor recurrence rate, but can＇t increase prostate tumor seeding.
