经腹腹腔镜与开放肾癌根治术的临床疗效比较

摘　　要：目的比较经腹途径腹腔镜肾癌根治术和开放肾癌根治术的治疗临床效果。方法行经腹腹腔镜肾癌根治术76例(A组),开放肾癌根治术48例(B组),比较两种方法在手术时间、术中出血量、术后肠功能恢复情况、术后下床活动时间、恢复正常工作时间、住院总费用、术后住院总天、术后止痛剂的用量、并发症等指标进行比较。结果 A组在手术中出血量、术后肠功能恢复情况、术后下床活动时间、术后住院总天数、恢复正常工作时间、止痛药用量、并发症方面明显优于B组,差异有显著性(均P<0.01);B组在手术操作时间、住院总费用优于A组,差异有显著性(P<0.05)。结论腹腔镜根治性肾切除术具有术中创伤小、术后恢复快、疼痛小的优点,对于T1～T2bN0M0期肾肿瘤,效果优于开放手术。[Objective] To summary the experience of laparoscopic radical nephrectomy and open radical nephrectomy.[Methods]From Jan 2000 to May 2009, we performed 76 （group A） retroperitoneal laparoscopic radical nephrectomy and 48 （group B） open radical nephrectomy were retrospectively analyzed. Operating time, amount of bleeding during operation, recovery of intestinal function after operation, ambulation and normal activities after operation , total cost of hospitalization, the hospital stay,analgesics use after operation and complications were compared between group A and group B.[Results]In group A, the amount of bleeding during operation, the recovery of intestinal function after operation, ambulation after operation, the hospital stay, the resume of normal activities after operation,analgesics use after operation and the occurrence of complications were all better than those in group B （P 0.01）, while in group B, the operation time, and the total cost of hospitalization were less than group A （P 0.05）. Laparoscopic radical nephrectomy is superior to open radical nephrectomy with less blood loss, faster recovery and less pain and similar prognosis. [Conclusions]Laparoscopic radical nephrectomy has the advantages of rapid recovery and less pain. Laparoscopic radical nephrectomy is superior to open nephrectomy for renal tumor of T1～T2bN0M0.
