老年妇女子宫内膜癌44例分析

摘　　要：目的探讨老年妇女子宫内膜癌的临床与病理特征。方法对１９８６年至１９９６年在我院治疗的１２８例子宫内膜癌中≥６０岁的４４例进行回顾性分析，并与同时期≤５０岁的３３例子宫内膜癌做对照分析。全部手术切除标本经病理检查确诊。按ＦＩＧＯ分期标准进行分期。结果≥６０岁组与≤５０岁组临床分期分别为：Ⅱ级以上的占４０９％（１８／４４例）和３３３％（１１／３３例）；组织分级Ｇ２－３分别为６３６％（２８／４４例）和４５５％（１５／３３例）；子宫肌层侵肌深度大于１／２分别为６３６％（２８／４４例）和３３３％（１１／３３例）（Ｐ＜００１）。５年存活率分别为５６３％和８２４％（Ｐ＜００１）。结论≥６０岁妇女子宫内膜癌通常临床分期晚、组织分化差、侵肌深、预后差。Objective To investigate the clinicopathologic characteristics of senile endometrial carcinoma.Methods 44 cases of endometrial carcinoma,older than 60 years(group one),were analyzed retrospectively.33 cases,less than 50 years old (group two),treated during the same period served as controls.The diagnosis was made according to the pathologic results,and the stages were determined by the FIGO criterion.Results Among the patients of group one and group two,the percentages of FIGO staging Ⅱ～Ⅳ were 40 9%(18/44)and 33 3%(11/33);the cases of histologic grade 2 to 3 were 63 6%(28/44)and 45 5%(15/33);the proportions of myometrial invasion depth(>1/2)were 63 6%(28/44)and 33 3%(11/33)(P<0 01),respectively..The 5-year survival rates were 56 3% and 82 4%(P<0 01).Conclusions Cases older than 60 with endometrial carcinoma are usually associated with late clinical stage,poor histologic grade,deep myometrial invasion and poor prognosis. \ \
