摘  要:目的探讨MIF和TNF-α与非酒精性脂肪性肝炎(NASH)的关系及MIF与TNF-α在NASH形成过程中的作用。方法非酒精性脂肪肝(NAFLD)患者58例,其中非酒精性单纯性脂肪肝(NASFL)组30例,NASH组28例。正常对照组(NC)30例,选自健康体检者。血清肿瘤坏死因子-α采用放射免疫法,MIF采用ELISA法检测。结果①NASH组患者ALT、AST、TG、TC、LDL、BMI水平均高于正常对照组(P<0.01,P<0.05),HDL低于正常对照组(P<0.05);NASFL组患者TG、BMI水平均高于正常对照组(P<0.05),其它生化指标差异均无显著性(P>0.05);NASH组患者ALT、AST、TG、TC、LDL、BMI水平均高于NASFL组(P<0.01,P<0.05);②NASH组患者血浆MIF及TNF-α水平均高于NASFL组及正常对照组,差异具有统计学意义(P<0.01);NASFL组患者血浆MIF及TNF-α水平与正常对照组比较,差异无统计学意义(P>0.05);③血浆MIF与ALT、AST、TG、BMI、TNF-α呈正相关(P<0.05);血浆TNF-α与ALT、AST、TG、BMI、MIF呈正相关(P<0.05);④以全组血浆MIF和TNF-α为因变量进行逐步回归,BMI对血浆MIF影响最大,血浆MIF含量变异的37.5%可由BMI、TG的变化来解释(R2=0.375)。BMI对血浆TNF-α影响最大,血浆TNF-α含量变异的41.7%可由BMI、TG、ALT的变化来解释(R2=0.417)。结论非酒精性脂肪性肝炎患者血浆MIF及TNF-α两者可能协同作用参与并促进了非酒精性脂肪性肝炎的发生、发展。Objective To investigate the relationship of blood plasma MIF,TNF-α and NASH,and the effect of MIF and TNF-α in the forming procedure of nonalcohalic steatohepatitis(NASH).Methods Fifty-eight patients with nonalcoholic fatty liver disease simple fatty liver(NAFLD) were divided into two groups,nonalcoholic simple fatty liver(NASFL) group and NASH group.The first group contained 30 patients,the second contained 28 patients.Normal control group contained 30 people,choosen from health examination.TNF-α was measured by radio immunoassay.MIF was measured by ELISA.Results①Levels of ALT、AST、TG、TC、LDL and BMI measured in NASH group were higher than those in normalcontrol group(P〈0.01,P〈0.05),HDL was lower than normal control group(P〈0.05);levels of TGand BMI in NASFL group were higher than those in normal control group(P〈0.05),other biochemical indicators had no statistical significance;ALT、AST、TG、TC、LDL and BMI in NASH group were higher than those in NASFL group(P〈0.01,P〈0.05);②MIF and TNF-α levels in NASH group were higher than those in NASFL and normal control group;and there was a statistical significance(P〈0.01).Therewas no statistical significance of MIF and TNF-α levels between the NASFL group and the normal control group(P〉0.05);③It was shown positive correlation of MIF and ALT,AST,TG,BMI,TNF-α (P〈0.05),and It was also shown positive correlation of TNF-α and ALT,AST,TG,BMI,MIF(P〈0.05);④The full set of blood plasma was used as dependent variable and a stepwise regression was done.BMI had the biggest influence to MIF,37.5% MIF level variant of blood plasma could be ex-plained by BMI and TG changing(R^2=0.375).BMI had the biggest influence to TNF-α,41.7% TNF-α level variant of blood plasma could be explained by BMI,TG and ALT changing(R^2=0.417).Conclusion The levels of MIF and TNF-α in blood plasma of the patient with NASH may have a synergisticeffect,and accelerate the happening and development of NASH.
