自体细胞因子诱导的杀伤细胞回输联合顺铂介入治疗中晚期宫颈癌的临床研究

摘　　要：目的评价自体细胞因子诱导的杀伤细胞(cytokine induced killer cell,CIK细胞)回输联合顺铂介入治疗中晚期宫颈癌的近期疗效。方法 38例中晚期宫颈癌患者随机分为自体CIK细胞回输联合顺铂介入治疗组(联合治疗组)及单纯介入治疗组(单纯治疗组)。治疗后观察患者疗效、生存质量、TH1/TH2免疫指标的变化。另选20例健康体检者为对照组,进行TH1/TH2等免疫指标的比较。结果完全缓解率在联合治疗组较单纯治疗组显著提高,分别为35%和22%(P<0.05);联合治疗组与单纯治疗组的生活质量总提高率分别为85.0%,61%(P<0.05);两组TH1/TH2治疗前无明显差异,但均低于对照组(P<0.05),治疗后联合治疗组较单纯治疗组有明显回升(P<0.05)。两组治疗前B细胞、自然杀伤细胞(natural killer cell,NK细胞)含量比较无显著性差异,联合治疗组治疗后1月与治疗前比较,NK细胞改善(P<0.05),且优于单纯治疗组(P<0.05)。结论自体CIK细胞回输联合顺铂介入治疗对中晚期宫颈癌具有较好的近期疗效,使其完全缓解率和生活质量有了较大提高,免疫情况有所改善。Objective To study the near future effect of interventional treatment combined with cytokine induced killer cell（CIK） for cervical carcinoma with DDP.Methods Thirty-eight cases of cervical carcinoma were randomly divided into 2 groups： treated with interventional therapy only or combined with CIK.Among them,the uterine arteries were embolized,and the curative effect,life quality and the change of TH1 /TH2 were observed and evaluated.Results The remission rate of interventional treatment combined with CIK was 35% and the effective rate in the near future was 85%,interventional treatment was 22% and the effective rate in the near future was 61%.Before treatment,there was no difference in TH1 /TH2 between the two groups,and they were all lower than that in the control group（P〈0.05）;after treatment,TH1 /TH2 was obviously higher in combined group than that in single group（P〈0.05）.There was no difference in content of B cells and NK cells before treatment;one month after treatment,content of NK cells was higher in combine group than that in single group,and they were all higher than that in the control group（P〈0.05）.Conclusion CIK combined with interventional treatment with DDP is effective and can improve the remission rate,life quality.
