大剂量5-FU持续96小时静脉输注联合方案治疗晚期头颈部鳞癌55例报告

摘　　要：目的评价大剂量５－ＦＵ（４０００ｍｇ／ｍ２）持续９６小时静脉输注联合方案对晚期头颈部鳞癌的疗效和不良反应。方法采用上述５－ＦＵ联合方案治疗晚期头颈部鳞癌５５例，其中联合ＤＤＰ＋ＰＹＭ（ＰＰＦ方案）３５例，联合ＤＤＰ（ＰＦ方案）２０例。结果完全缓解（ＣＲ）１１例，部分缓解（ＰＲ）２４例，稳定（Ｓ）１１例，进展（Ｐ）９例，有效率（ＣＲ＋ＰＲ）６３．６％。全组中位生存期１３．１月。不良反应主要为恶心、呕吐（８５．５％），白细胞降低（３６．４％），血小板降低（９．１％），口腔粘膜溃疡及腹泻（６０．０％），外周静脉炎（６９．１％）等。结论本方案对头颈部鳞癌有较好疗效，骨髓抑制轻。Objective To evaluate the efficacy and the toxicities of combination chemotherapy regimen containing 96 hours high dose 5 FU continuous infusion.Method Fifty five cases of advanced squamous cell carcinoma of the head and neck were treated with combination chemotherapy containing continuous high dose 5 FU infusion.Among all cases 35 were combined with DDP+PYM (PPF regimen),20 with DDP alone.Result Eleven cases achieved CR,24 PR,11 S and 9 P.The total reponse rate (CR+PR) was 63.6% .The median survival term of the patients was 13.1 months.The toxicities observed were nausea and vomiting (85.5%),leucopenia (36.4%) and thrombocytopenia (9.1%),mucitis of the oral cavity and diarrhea (60.0%),and peripheral phlebitis (69.1%).Conclusion The regimen show satisfactory efficacy for advanced cell carcinoma of the head and neck with mild myelosuppresion,but mucitis of digestive tract and the perpheral phlebitis are more serious.
