摘 要:目的制作兔子宫内膜VX2肿瘤模型并探讨其在介入放射学研究中的应用价值。材料与方法新西兰大白兔30只采用宫腔内瘤块置入法建立肿瘤模型,分为实验A组(n=10)、B组(n=10)及C组(n=10),经右侧颈动脉用3F导管选择插至子宫动脉内分别注入顺铂(0.5mg/kg)、聚乙烯醇(PVA)颗粒及生理盐水(1ml/kg),分别于种植后2周和介入治疗后1周行MRI平扫及增强扫描观察肿瘤生长及坏死情况,处死观察其肿瘤大体外观并行病理学检查。结果 30只大白兔种植2周后经过MRI检测证实子宫均有肿瘤生长,种植成瘤率100%。在行介入治疗时有2只死于出血,1只死于术后感染。MRI平扫表现为子宫腔内占位性病变,呈类圆形或分叶状等信号肿块,增强扫描呈中等或明显不均匀强化,或呈边缘强化;血管造影示动脉期子宫动脉增粗,实质期肿瘤染色明显。A组和B组肿瘤治疗前后大小之间无统计学意义(P>0.05),C组治疗前后及C组与A、B组治疗后大小与坏死百分比之间的差异具有统计学意义(P<0.05)。兔子宫病理切片HE染色示兔子宫内膜腺体下鳞癌组织浸润。结论兔子宫内膜VX2肿瘤模型建立容易,成功率高,适合介入治疗,是一种较好的研究兔子宫内膜肿瘤的介入治疗模型。MRI能对兔子宫内膜VX2肿瘤进行动态观察,其表现具有一定特征性。Objective To establish the rabbit model bearing VX2 tumor and to investigate the efficiency of interventional treatment.Materials and Methods Thirty New Zealand white rabbits were divided into 3 groups(group A,B and C with 10 rabbits for each group) randomly.The rabbit model was established by implantating VX2 tumor fragments into uterine cavity.DDP(0.5mg/kg),PVA and saline(1ml/kg) were injected through right carotid artery for group A,B and C respectively.Plain and contrast-enhanced MR scan were performed to observe the tumors growth at two weeks after implantation and the tumor necrosis at one week after interventional treatment.Subsequently,the rabbits were sacrificed and the tumors were taken out for pathological examination.Results The tumors were successfully implanted in thirty rabbits uterine cavity with the success rate of 100%.Two rabbits died of hemorrhage and one died of infection after interventional treatment.Round or lobulated neoplasms with isointensity signal were displayed in rabbits uterine cavity on MR plain images.The tumors showed moderate to obviously heterogeneous enhancement or peripheral enhancement on contrast-enhanced images.Angiography showed uterine artery dilated and tumor stain.There was no significant difference of tumor volume change before and after interventional treatment between groupe A and B.There was significant difference of tumor volume and necrosis percentage before and after interventional treatment in goupe C.There was significant difference of the tumor volume and necrosis between groupe C and A or between group C and B.HE stain showed that there was some squamous cell carcinoma tissue under endometrium gland.Conclusion It's convenient and feasible to establish the rabbit VX2 uterus tumor model.The tumors can be observed effectively using MRI,and it is a good model for study of VX2 uterus tumor and fit to interventional treatment.
