三乙醇胺乳膏预防头颈部肿瘤放射性皮肤损伤的临床观察

摘　　要：目的观察三乙醇胺乳膏对预防头颈部肿瘤患者放疗致急性放射性皮肤损伤的临床疗效。方法将54例需要放疗的头颈部肿瘤患者随机分为2个组,28例在第1次放射治疗后予以三乙醇胺乳膏涂沫的为用药组,26例只做常规放射治疗宣教和健康教育的为对照组。结果两组皮肤反应发生率均为100%。重度放射性反应(Ⅲ+Ⅳ级)用药组为10.7%(3/28),对照组为46.2%(12/26)。皮肤反应发生剂量出现于照射40 Gy以前:用药组32.1%(9/28)、对照组96.1%(25/26),两组具有明显的统计学差异(P<0.05)。结论三乙醇胺乳膏能有效的降低头颈部肿瘤放射性皮肤损伤的程度,推迟损伤出现的时间,保证放射治疗的顺利进行。Objective To observe the clinical effect of trolamine cream to prevent acute radiation-induced dermal injury in patients with head-neck carcinoma.Methods 54 patients with head-neck carcinoma treated by radiotherapy were randomized into two groups,Patients in the treatment group（28 cases） were given trolamine cream since the first day of radiotherapy and patients in the control group（26 cases） were given routine health education.Results Dermal toxic rate of the two groups were 100%.The rates of acute radiation dermatitis of grade（Ⅲ＋Ⅳ） in the treatment group and in the control group were 10.7%（3/28） and 46.2%（12/26）,respectively,acute radiation occurred before 40 Gy were 32.1%（9/28）and 96.1%（25/26）.There was significant difference between the two groups（P〈0.05）.Conclusion Trolamine cream can effectively prevent acute radiation-induced dermal injury in patients with head-neck carcinoma.It can alleviate the patients suffering and improve their quality of life,so as to ensure uneventful radiotherapy.
