37例胃肠道间质瘤的临床诊治分析

摘　　要：目的探讨胃肠道间质瘤(GIST)的临床表现特点和治疗方法。方法对37例胃肠道间质瘤患者的临床资料进行回顾性分析。结果 37例中有34例有明显的临床症状(91.9%),19例因消化道出血就诊,18例因发现腹部肿块或有腹痛、腹胀等不全性梗阻症状就诊,3例无明显临床症状,体检时发现。37例患者均行手术治疗,CD117阳性37例(100%)。结论早期较难诊断,通过病理组织学及免疫组化检查可明确本病的诊断,手术是主要的治疗方法。对于有脏器转移者应高度重视。Objective To explore the diagnosis and treatment of gastrointestinal stromal tumors.Methods Thirty-seven patients with gastrointestinal stromal tumors from Oct 2007 to Oct 2009 were retrospectively analyzed.Results 34 of 37 patients（91.9%）had obvious clinical symptoms,19 cases were treated due to gastrointestinal bleeding.18 cases had abdominal mass or abdominal pain,they were found by abdominal distension and other symptoms of incomplete obstruction;three cases of no obvious clinical symptoms,which were found by physical examination.Resection was performed in 37 cases.CD117 was positive in 37 cases（100%）.Conclusion The gastrointestinal tromal tumor was rare and its diagnosis was difficult due to the lack of clinical feature.Final diagnosis should be based on pathology and immunohistochemistry.Surgery was the main treatment.Metastasis should have a high degree of attention.
