奈达铂联合替加氟治疗晚期食管癌观察

摘　　要：目的:探讨国产奈达铂(NDP)联合替加氟(TF)方案治疗晚期食管癌的近期疗效和毒副作用。方法:2006年12月～2008年12月在我院治疗的食管癌患者72例,分为治疗组与对照组;治疗组36例采用NDP+TF方案,NDP 80mg/m^2,静滴,第1天,TF 750/m^2,静滴,第1天～5天,每28天为1个周期,2周期为1个疗程;对照组36例采用DDP+5-Fu方案,DDP 80mg/m^2,静滴,第1天,5-Fu750mg/m^2,静滴,第1天～5天,每28天为1周期,2周期为1个疗程。结果:全部患者均可评价疗效及不良反应,其中NDP+TF方案治疗36例,临床有效率为72.2%,DDP+5-Fu方案治疗36例,临床有效率为47.2%,两组患者疗效比较差异有统计学意义(χ~2=4.677,P<0.05,),恶心、呕吐较对照组明显减轻(χ~2=5.675,P<0.05),骨髓抑制治疗组较对照组明显,差异有统计学意义(P<0.05)。结论:奈达铂联合替加氟治疗晚期食管癌近期疗效较DDP联合5-Fu方案佳,毒副反应可耐受。Objective： To explore short-term efficacy and toxicity of the domestic nedaplatin （NDP） combined with tegafur （TF） in the treatment of advanced esophageal carcinoma. Methods： 72 cases with advanced esophageal carcinoma were divided into treatment group and control group. In treatment group, 36 cases were treated by NDP combined with TF regimen： NDP 80mg/m^2 , intravenous infusion, dt ;TF 750/m^2 , intravenous infusion, d1.5 , every 28 days for a cycle;in the control group, 36 cases were treated by DDP combined with 5- Fu regimen： DDP 80mg/m^2 , intravenous infusion, d1 ,and 5-Fu750mg/m^2, intravenous infusion, d1.5 , every 28 days for a cycle ; two cycles for one treatment course. Results： The efficacy and adverse reactions were evaluable in all cases. In treatment group, effective rate was 72. 2%, and in control grcup was 47.2%. There was statistically significant difference between two groups in efficacy （Х^2 = 4. 677 ,P 〈 0.05 ）. Myelosuppression in theatment group was severer than that in control group. Nausea/vomiting reaction was milder in treatment group than that of control group, （Х^2 = 5.675, P 〈 0. 05 ）. Conclusion： Short-term efficacy of nedaplatin combined with tegafur in treatment of advanced esophageal carcinoma is superior to 5-Fu combined with DDP, with slight nausea and vomiting reactions. It is worthy of further research in clinical work.
