摘  要:目的探讨左旋四氢巴马汀(L-THP)对脑缺血再灌注损伤的保护机制。方法采用大脑中动脉阻塞法建立大鼠局灶性脑缺血再灌注模型。将90只SD大鼠随机分为①空白对照(control),②假手术组(shame),③模型组(I-R),④左旋四氢巴马汀大剂量组(I-R+L-THP50mg/kg),⑤左旋四氢巴马中剂量组(I-R+L-THP25mg/kg),⑥左旋四氢巴马汀小剂量组(I-R+L-THP12.5mg/kg),每组又分为缺血3h再灌注3、6、24h。每组15只SD大鼠,每时间点5只SD大鼠。采用11分法分别对大鼠进行行为缺陷评分;采用免疫组织化学法测定核转录因子(NF)-κB,细胞黏附分子(ICAM)-1的表达。结果 I-R组各时间点神经行为学评分、NF-κB,ICAM-1的表达均增加,数值6h较3h增高,24h最高,与3h相比差异均有统计学意义(P<0.05或P<0.01)。L-THP各剂量组能明显改善上述指标,L-THP大、中剂量与I-R组各时间点相比,P均<0.01或0.05。结论在脑缺血再灌注损伤24h内,以24h损伤更为严重。L-THP对脑保护作用可能与抑制炎症反应有关。Objective To study the protective mechanism of L-tetrahydropalmatine(L-THP)on local cerebral ischemia reperfusion injury in rats.Methods Modified Longa method was used to make the model of local cerebral ischemia reperfusion injury in rats.Ninety healthy SD rats were randomly divided into six groups:①control group,②sham operation group,③ischemia reperfusion model group,④large dose L-THP group(I-R+L-THP 50 mg/kg),⑤middle dose L-THP group(I-R+L-THP 25 mg/kg),⑥small dose L-THP group(I-R+ L-THP 15 mg/kg).Each group was divided into ischemia3h,reperfusion3 h,6 h,24 h subgroup.Neurological deficit scores were measured,the expression of NF-κB,ICAM-1 will be measured with immunohistochemistry.Results The experiment indicated that the ischemia reperfusion injury resulted in high neurological deficit scores,high expression of NF-κB,ICAM-1.The signs are higher on 6 h than 3 h,and highest on 24 h.The results indicated that different doses L-THP could improve all signs mentioned above.But the effects of large and middle dose L-THP group are the best(P0.01 or P0.05).The low dose L-THP group have the better effects(P0.05).Conclusion The cell injury of cerebral ischemia reperfusion is severely on 24 h.The protective effects of L-THP on rat brain damage induced by local cerebral ischemia reperfusion might be related to reducing the damage of free radical.
