依达拉奉对急性脑出血患者ICAM-1和MMP-9的影响作用

摘　　要：目的观察依达拉奉对急性脑出血患者血清细胞间黏附分子-1(ICAM-1)和基质金属蛋白酶-9(MMP-9)表达的影响。方法将56例急性脑出血内科治疗患者随机分为常规治疗组26例和依达拉奉治疗组30例,统计两组患者治疗前和治疗后3、7、14d血清ICAM-1、MMP-9含量,并对脑出血患者在相应时间检查头部CT,计算脑水肿大小,对结果进行分析。结果治疗后3、7d,依达拉奉组患者血清ICAM-1、MMP-9含量显著低于常规治疗组(P<0.05);治疗后14d两组患者血清ICAM-1、MMP-9含量下降到大致相同水平(P>0.05)。治疗后3,7、14d,依达拉奉组脑水肿体积小于常规治疗组(P<0.05)。结论依达拉奉可显著降低急性脑出血患者ICAM-1和MMP-9的表达,减轻脑血肿周围脑组织炎症反应和继发性脑组织损伤,改善神经功能缺损,对脑出血患者起到保护作用。Objective To observe the changes of serum intercellular adhesion molecule-1 （ ICAM-1） and matrix metalloproteinase-9 （MMP-9） in patients with acute intracerebral hemorrhage （ICH）.Methods 56 cases of acute intracerebral hemorrhage were enrolled in this experiment.According to the usage of edaravone or not,they were divided into two groups：the routine group in which 26 cases were treated with routine therapy and the edaravone group in which 30 cases received edaravone injection besides routine therapy.ICAM-1 and MMP-9 were assayed at different time points （before therapy,3d,7d,and 14d after treatment）.The size of brain edema on computed tomographic （CT） scan in ICH patients was determined at the corresponding time.Results On 3d and 7d,the levels of serum ICAM-1 and MMP-9 in the edaravone group were lower than those in the routine group （P〈0.05）.ICAM-1 and MMP-9 levels of two groups were at the same level on 14d （ 〉0.05）.On 3d,7d and 14d,the sizes of brain edema in the edaravone group were smaller than those in the routine group （P〉0.05）.Conclusion Edaravone can decrease the levels of ICAM-1 and MMP-9 in patients with acute intracerebral hemorrhage.It can lighten acute inflammation around hematoma and brain injured,lessen nerve function impairment,and protect the tissue of brain.
