骨巨细胞瘤增殖细胞核抗原与bc1-2基因表达及凋亡的检测研究

摘  要:探讨骨巨细胞瘤(GCT)的细胞学性质、肿瘤发生以及生物学行为。方法用免疫组化(标记的链霉卵白素生物素──LSAB)法检测51例GCT中PCNA和bc1-2蛋白表达,并用TUNEL法检测其凋亡细胞。结果全部标本PCNA阳性表达,随病理分级增加,PCNA表达有增高趋势。bc1-2在Ⅰ与Ⅲ级及Ⅰ与Ⅱ+Ⅲ级间表达差异有显著性意义。GCT中多为单核基质细胞凋亡,GCT的凋亡率(AI)为0.86%,各级间及PCNA标记指数间AI的差异无显著性意义,GCT的AI与bc1-2表达呈显著负相关,bc1-2表达与否的AI差异有显著性。结论GCT是以增殖为特征的肿瘤;bcl-2在GCT发生发展中可能起一定的作用。Objective To study the features of cytology, oncogenesis and biological behavior of giantcell tumor (GCT) of bone. Method The expression of PCNA and bc1-2 protein was detected by using immunohistochemical LSAB technique and cell apoptosis using TUNEL method in 51 cases of GCT. Accordingto Jaffe's classification, 26 cases were classified in grade Ⅰ, 16 cases in grade Ⅱ, and 9 cases in grade Ⅲ.Result The expression of PCNA was positive in all samples, but PCNA no staining in multinuclear giantcells. The proportion of positive PCNA labeled cells tended to be more with a higher pathological grade. Thepositive rate of bcl-2 expression was 33. 3% in GCT with the differences between Ⅰ and Ⅲ orⅠ and Ⅱ+Ⅲgrade being statistically significant (P<0.05). Apoptosis mainly occurred in mononucleate stromal cells ofGCT, rarely in multinuclear giant cells. Apoptosis index (AI) of GCT was 0. 86%. No significant differencein the AI between pathological grades and between PCNA labeling index. There was a negative correlation between AI and positive expression of bc1-2 protein in GCT (P<0.05). There was a significant diffelence inthe. AI between bc1-2 positive and negative expression in GCT (P<0.05). Conclusion GCT is a tumorwith the proliferative characteristic. bcl-2 gene may play a role in the development of GCT.
