摘　　要：目的探讨慢性乙型肝炎(CHB)患者阿德福韦酯(ADV)抗病毒治疗前后ADV对机体免疫功能的影响。方法收集37例ADV抗病毒治疗有效的CHB患者治疗前、治疗3个月、6个月和1年时血清,ELISA检测IL-12、IFN-γ、IL-4和IL-10水平。结果 37例CHB患者ADV治疗后各时间点IL-12和IFN-γ水平均显著高于治疗前(P<0.05)和健康对照组(P<0.05),以治疗3个月时最高,后逐渐降低;IL-4和IL-10治疗后各时间点均显著低于治疗前(P<0.05),高于健康对照组(P<0.05),以治疗6个月时最低,1年时稍有增高,但无统计学意义(P>0.05)。结论 ADV治疗可促进CHB患者产生更多的TH1型细胞因子,使TH1/TH2比值趋于平衡,促进了机体免疫功能的增强和恢复。Objective To observe the dynamic changes of serum cytokines in patients with chronic hepatitis B（CHB） treated with adefovir dipivoxil（ADV）.Methods Thirty-seven CHB patients treated with ADV and 20 healthy subjects were included and followed up for one year.ELISA was used to measure the concentrations of IL-12,IFN-γ,IL-4,and IL-10 in sera collected before and month 3,6,and 12 after the treatment.Results ADV therapy was consistently effective in the 37 patients.Compared with those before therapy,the levels of IL-12 and IFN-γ at different times after therapy were all significantly up-regulated（P0.05）.IL-12 and IFN-γ reached to highest at month 3 after ADV treatment,and then decreased gradually,yet they were higher than those in the healthy controls（P0.05）.The levels of IL-4 and IL-10 at different times after therapy were all significantly lower than those before therapy（P0.05）,and were lowest at month 6,then slightly increased at month 12（P0.05）.Conclusions ADV therapy can make CHB patients produce more Th1 cytokines.The immunity of CHB patients can be enhanced and restored to some extent by ADV.
