选择性拆线与否对人工晶状体术后角膜散光的影响

摘　　要：目的观察人工晶状体术后角膜散光的长期演变以及选择性拆线与不拆线对其的影响。方法人工晶状体手术患者８３例（９６眼），根据术后６周时角膜曲率检查结果分为拆线组及不拆线组，分别测量两组患者术前、术后６周；３，６，１２及２４个月时的角膜曲率。结果术后３，６个月时拆线组的平均角膜散光度较不拆线组显著降低，且转为逆规性散光者多。术后１２～２４个月时，两组的平均角膜散光度无明显区别，逆规性散光的比例分别为６０．７８％及６０．００％。结论选择性拆线可减少术后短期内角膜循规性散光的度数，并促使其向逆规性方向转化，但对术后远期结果无影响。ObjectiveTo observe the long term changes of corneal astigmatism following IOL surgery and results following selective suture removal or nonremoval.Methods83 cases (96 eyes) accepted for IOL implantation were divided into suture removal group and non removal group according to the corneal astigmatism degree at 6 weeks postoperation. Corneal curvatures were measured preoperation and on 6 weeks,3,6,12 and 24 months postoperation.ResultsCompared with the control group, the mean astigmatism degrees decreased significantly after suture removal at 3 and 6 months postoperation;however, the astigmatism conditions between both groups were similar on 12～24 months postoperatively.ConclusionSelective suture removal may decrease the degree of corneal astigmatism with the rule in short term after IOL surgery and accelerate the transformation to an astigmatism condition against the rule. It has no effect on long-term corneal degree and conditions of postoperation.
