摘  要:目的:应用营养不良通用筛查工具(MUST)对胃癌术前营养风险进行评分,观察其评估特点及其对胃癌手术治疗的影响。方法:收集广州中山大学附属第三医院2004-2007年手术治疗的314例胃癌患者,依照中国体质指数正常值的MUST评估标准进行术前营养风险评分,观察其对术后并发症和住院天数的影响。结果:经MUST标准筛查,低、中和重度营养风险人数分别为147例(46.8%)、65例(20.7%)和102例(32.4%)。低、中和重营养风险胃癌患者出现围手术并发症人数分别为14例(9.5%)、9例(13.8%)和26例(25.4%),3组比较差异有统计学意义,P<0.05。低、中和重营养风险胃癌患者平均住院天数分别为(14.38±8.19)、(15.68±8.87)和(18.17±11.34)d,重营养风险胃癌患者平均住院天数高于低营养风险胃癌患者(P<0.05)。利用多元logistics回归分析,术前营养评分发生术后并发症的OR值为-0.418(P<0.05)。结论:术前营养评估为重营养风险预示胃癌患者围手术期更易发生并发症和住院时间长,此类患者术前需加强营养支持。OBJECTIVE:To evaluate the nutritional risk of patients with gastric carcinoma using the malnutrition universal screening tool(MUST)and its relationship with postoperative results.METHODS:The consecutive 314 gastric cancer patients from 2004 to 2007 were evaluated for the nutritional risk with MUST,in accordance with China's normal body mass index(BMI).The relationship between postoperative complications,days in hospital and MUST was observed.RESULTS:The number of patients with low,median and high nutritional risk by MUST were 147 cases(46.8%),65 cases(20.7%)and 102 cases(32.4%)respectively.The number of patients with postoperative complications in the low,median and high nutritional risk groups were 14(9.5%),9(13.8%)and 26(25.4%)respectively,and the difference was significant(P〈0.05).The lengths of stay in hospital in the low,median,high nutritional risk groups were(14.38±8.19),(15.68±8.87)and(18.17±11.34)d respectively,and the length of stay in hospital in the high nutritional risk group was longer than that in the low nutritional risk group(P〈0.05).The odds ratio to develop a complication was-0.418 in patients with MUST screening(P〈0.05).CONCLUSIONS:High nutritional risk foreshadows the more postoperative complications and longer length of stay in hospital.It indicats that preoperative nutritional support is necessary for patients with high nutritional risk.
