摘　　要：目的:总结直肠癌术前放疗的研究进展。方法:应用检索Medline及CNKI期刊全文数据库检索系统,以"术前放射治疗和直肠肿瘤"为关键词,检索1990-01-2009-10的相关文献,共检索到文献1200条。纳入标准:1)关于直肠癌术前放疗的适应证、优缺点、放疗模式的研究;2)关于直肠癌术前放疗的意义、不良反应、术前放疗敏感性预测、术前放化疗方面的研究。根据纳入标准,最后分析34篇纳入。结果:直肠癌术前放疗能使肿瘤缩小,提高肿瘤切除率,提高保肛率,减少复发;并且术前放疗毒副反应轻。术前放化疗与术前放疗相比,可进一步降低局部复发率,降低分期。结论:术前放疗在直肠癌新辅助治疗中扮演重要角色。关于术前精确分期、放疗方式的选择、以及术前放疗敏感性预测方面的问题将是我们亟待解决的。OBJECTIVE：To review the research progress in preoperative radiotherapy for rectal cancer.METHODS：A total of 1 200 papers were searched with preoperative radiotherapy and rectal neoplasms as key words in Medline and CNKI databases from 1990-01 to 2009-10,and 34 papers were selected according to the included criteria：1）papers about rectal cancer indications of preoperative radiotherapy,advantages and disadvantages,and radiation pattern studies;2） papers about the significance of preoperative radiotherapy,side effects,predicting the sensitivity of preoperative radiotherapy,and preoperative chemoradiotherapy studies.RESULTS：The preoperative radiotherapy can shrink tumor volume,improve the resection rate of tumor,improve sphincter preservation rates,and reduce recurrence.The side effects of preoperative radiotherapy are mild.Compared with preoperative radiotherapy,preoperative chemoradiotherapy can further reduce the local recurrence rate and lower staging.CONCLUSIONS：Preoperative radiotherapy plays an important role in neoadjuvant therapy for rectal cancer.The problem how to stage precisely,select patterns of radiotherapy,as well as predict the sensitivity of preoperative radiation needs to be resolved urgently.
