64层螺旋CT血管成像对肝癌介入治疗的价值

摘　　要：目的 探讨64层螺旋CT肝脏三期扫描肝动脉三维重建对肝癌介入治疗的指导意义.方法 53例中晚期原发性肝细胞癌患者介入治疗前行64层螺旋CT肝脏三期扫描,分析肝脏肿瘤肝内、外供血动脉三维重建的表现,并与血管造影进行对比.结果 所有肝动脉期VRT图像成像均能清晰显示腹主动脉及其重要分支的起源、走行.其中肝动脉血管变异24例,肿瘤由肝右动脉及肝左动脉双重供血9例,肝外供血动脉37条.肿瘤肝内、外供血动脉显示与血管造影一致.结论 64层螺旋CT肝脏血管三维重建对指导制订介入治疗方案具有较高实用价值.Objective To investigate the guiding significance of 64--slice spiral CT angiography in interventional therapy of hepat ic carcinoma. Methods 53 cases with advanced primary hepatocellular carcinomas underwent 64-- slice spiral CT tri--phase scan ning,the intra--hepatic and extra--hepatic feeding arteriae of liver tumors were analysed comparatively with DSA at three--dimen sional reconstruction. Results Volume rendering technology（VRT）images could clear display the origins and arranges of the abdom inal aorta and it＇s important branches. Hepatic arterial variation was found in 24 cases, the tumors were supplied by both right hepat ic artery and left hepatic artery in 9 cases, there were 37 extra--hepatic feeding arteriae,which were corresponded to that of angiogra phy. Conclusion The three--dimensional reconstruction of 64--slice spiral CT angiography is of practical value for guiding interven tional therapy.
