肝癌肝移植术后肝癌复发大鼠模型的建立

摘　　要：目的通过在大鼠肝癌肝移植后免疫抑制状态下接种肿瘤组织块来模拟临床中同种异体肝移植后肝癌复发的进程,建立稳定的肝移植后肝癌复发模型。方法应用Wistar大鼠肝癌细胞系CBRH-7919经过培养传代、皮下成瘤、肝内接种等过程建立大鼠的种植性肝癌模型。行SD→Wistar的大鼠移植,免疫抑制状态下肝内植入瘤块。A组(n=15),以免疫抑制状态下的未行肝移植的Wistar大鼠肝内植瘤,作为对照组;B组(n=15),为肝移植术中植瘤组;C组(n=15),为术后1周植瘤组。观察各组大鼠的成瘤率,在B超下观察肿瘤生长情况,绘制生长曲线。结果肝移植术中植瘤组(B组)大鼠仅6只成瘤,成瘤率为40.0%,而对照组(A组)和术后植瘤组(C组)大鼠各有13只成瘤,成瘤率为86.7%。术中植瘤组大鼠肿瘤生长速度明显降低。结论肝移植术后1周在免疫抑制状态下肝内接种瘤块成瘤,建立了安全、可靠的肝移植术后肝癌复发的大鼠模型。Objective To explore a new way to develop a rat model of tumor recurrence in liver allograft with intrahepatic implantation of tumor fragments and the use of immunosuppressant,study tumor recurrence and tumor growth in liver grafts in recipients with HCC under well-defined conditions.Methods CBRH-7919 cell line,which is syngenic to Wistar rats,were cultured in vivo,then implanted subcutaneously in the Wistar rats and serially passaged prior to implantation in the liver.Small tumor fragments were implanted un-der the liver capsule.The tumor size was measured with ultrasound and the tumor volume was calculated.Group A（n=15）,a control group,a tumor was implanted and measured in Wistar livers.Group B（n=15）,a SD→Wistar allogenic ROLT was performed and a fresh tumor piece was implanted after reperfusion.Group C（n=15）,a tumor was implanted in the liver 7 d after ROLT.Tumor growth curves were compared.Results Tumor was taken in thirteen of fifteen rats in group A,in six of fifteen rats in group B,and in thirteen of fifteen rats in group C.A significant growth retardation of the tumor was seen in group B.No difference in growth rate was observed between group C and group A.Conclusion Tumor implantation in the allograft liv-er in the 7th day after rat orthotopic liver transplantation under immunosuppression establishes a safe and reli-able rat model of tumor recurrence in liver graft.
