鼻腔鼻窦罕见非上皮组织恶性肿瘤的诊治探讨

摘　　要：目的讨论临床鼻腔鼻窦罕见非上皮组织恶性肿瘤的临床特征、诊断及治疗。方法对１９８６～１９９８年诊治鼻腔鼻窦非上皮组织恶性肿瘤１６例分类，就组织切片、临床诊断和治疗进行回顾性分析。结果鼻腔鼻窦恶性淋巴瘤３例，髓外浆细胞瘤２例，纤维肉瘤２例，胚胎性横纹肌肉瘤１例，恶性黑色素瘤３例，恶性混合瘤１例，恶性肉芽肿４例。综合治疗后随访，８例存活３～５年，其中恶性淋巴瘤２例，恶性黑色素瘤１例，髓外浆细胞瘤２例，恶性肉芽肿３例。结论此类肿瘤特点：①不易明确诊断，易与其它肿瘤相混淆。②多易复发及全身转移。③临床病理学诊断及组织学分类较困难，④综合治疗有益于改善预后。Objectives To study the clinical features and diagnosis and treatmant of rare nonepithelium neoplazma in nasal cavity and paranasal sinus.Methods 16 cases of nonepithelium neoplazma in nasal cavity and paranasal sinas were diagnosed and treated from 1986 to 1998.The pathologic findings in each case were reviewed carefully and correlated with clinical diagnosis and treatment. Results lymphoma was in 3 cases,extramedullary plasmacytomo in 2 cases,fibrosarcoma in 2 cases,embryonal rhabdomyosarcoma in 1 case,melenoma in 3 cases,malignant mixed neoplazma in 1 cases,malignant granuloma in 4 cases.Following-up after synthetic treatment 8 cases survived for 3～5 years,2 cases in lymphoma,1 case in melenoma,2 cases in extramedullary plasmacytome,3 cases in malignant granuloma.Conclusions This kind of neoplazma show following primary features:①not easy to diagnose definitely and mixed otten with ofher neoplazma.②easy to recurrent and metastasis.③difficult to marster in pathologic diagnosis and tissue classification.④beneficial to improve prognosis with synthetic treatment.
