摘  要:目的探讨"纯"抑郁患者蒙特利尔认知评估量表(MoCA)评估的认知功能改变。方法选取首次发作的"纯"抑郁患者,经汉密尔顿抑郁量表17(HAMD17)评定抑郁程度后分为轻度抑郁组(7~17分,41例)、中重度抑郁组(>17分,30例),进行MoCA认知功能评定,并与正常人26例对照。结果中重度抑郁组的MoCA总分和部分因子分均显著低于轻度抑郁组和对照组(P<0.05),后两者间差异无统计学意义。逐步线性多重回归分析显示抑郁程度、年龄和受教育年限均显著影响认知功能,程度依次递减。HAMD17各因子中焦虑因子对认知功能影响最大。结论中重度抑郁可损害认知功能,且主要源自焦虑。Objective To explore cognitive function assessed by Montreal Cognitive Assessment Scale(MoCA)in patients with "pure" depression.Methods The patients were single major depressive episode.Following an assessment of depressive severity by Hamilton Depression Rating Scale 17(HAMD17),the patients were divided into two groups:mild depression(7~17 scores,41 cases),moderate to severe depression(more than 17 scores,30 cases),and their cognitive function was evaluated by Montreal Cognitive Assessment Scale(MoCA).Results In the moderate-severe depression group the MoCA's total score and some factor scores were significantly lower compared to the mild depression group and the control group respectively(P〈0.05),there was no significant difference between mild depression group and the control group.Multiple linear regression analysis showed that the factors impaired cognition were depressive severity,age and years of education,with anxiety being the strongest influential factors among all factors of HAMD17.Conclusion Moderate or severe depression could affect cognitive function and anxiety factor was dominant.
